头针治疗血管性痴呆随机对照研究

摘　　要：目的研究头针治疗血管性痴呆(VD)与西药治疗的疗效差别,探讨头针治疗VD的优势。方法将符合纳入标准的80例VD患者随机分为头针组和药物组,每组40例。头针组采用头针疗法,每日针刺1次,14 d为1个疗程,共治疗2个疗程。药物组采用胞二磷胆碱注射液,连续静脉滴注,14 d为1个疗程,共治疗2个疗程。比较两组患者临床疗效﹑SF-36健康生活质量评分、简易精神状态检查量表(MMSE)评分﹑日常生活自理能力量表(ADL)评分。结果头针组总有效率为94.7%,明显高于药物组(P<0.05)。头针组治疗后SF-36健康生活质量评分高于药物组(P<0.05),头针组治疗后MMSE﹑ADL评分与药物组比较差异均有统计学意义(P<0.01)。结论头针治疗VD明显改善了患者的临床症状﹑认知功能,提高了患者的生活质量,其总体疗效较药物组具有优势。Objective To study the difference in therapeutic efficacy between scalp acupuncture and Western medication for vascular dementia （VD）, and to discuss the advantage of scalp acupuncture in treating VD. Method Eighty eligible VD patients were randomized into a scalp acupuncture group and a medication group, 40 in each. The scalp acupuncture group received scalp acupuncture once each day, 14 d as a treatment course, and 2 courses in total. The medication group received Citicoline injection via intravenous infusion, 14 d as a treatment course, and 2 courses in total. Clinical efficacy, SF-36 scale for quality of life, Mini-Mental State Examination （MMSE）, and Activity of daily Living Scale （ADL） were adopted to compare the two groups. Result The total effective rate was 94.7% in the scalp acupuncture group, which was significantly higher than that in the medication group （P〈0.05）. After treatment, SF-36 score in the scalp acupuncture group was markedly higher than that in the medication group （P〈0.05）; the differences in MMSE and ADL scores between the two groups were statistically significant （P〈0.01）. Conclusion Scalp acupuncture improves clinical symptoms, cognitive function, and life quality of VD patients, and it has advantages in general therapeutic efficacy compared to medication.
