血清MMP-2、TIMP-1及hs-CRP与2型糖尿病颈动脉粥样硬化的相关性研究

摘　　要：目的探讨血清基质金属蛋白酶-2(MMP-2)、组织抑制因子-1(TIMP-1)及高敏C反应蛋白(hs-CRP)与2型糖尿病(T2DM)患者颈动脉粥样硬化的关系。方法选择62例T2DM患者作为研究对象,测量患者颈动脉内中膜厚度(IMT)、斑块情况。根据IMT值将选取T2DM患者分为3组:单纯糖尿病组21例(糖尿病组),糖尿病颈动脉内中膜增厚组21例(中膜增厚组),糖尿病颈动脉粥样硬化斑块形成组20例(斑块组);另选健康查体者20名作为正常对照组。测定血清MMP-2、TIMP-1及hs-CRP水平并进行相关分析。结果与正常对照组相比,3组糖尿病患者血清MMP-2、hs-CRP水平升高,TIMP-1降低,差异有统计学意义(P<0.05或P<0.01);与单纯糖尿病组比较,中膜增厚组MMP-2、hs-CRP水平升高,TIMP-1降低,差异有统计学意义(P<0.05);斑块组MMP-2、hs-CRP水平明显高于单纯糖尿病组和中膜增厚组,TIMP-1低于单纯糖尿病组和中膜增厚组,差异有统计学意义(P<0.05或P<0.01)。结论血清MMP-2、hs-CRP升高及TIMP-1降低与颈动脉粥样硬化的病变程度密切相关。Objective To investigate the relationship between matrix metalloproteinase-2（MMP-2）,tissue inhibitor of metalloproteinase-1（TIMP-1） and hs-CRP in type 2 diabetes mellitus（T2DM） with carotid atherosclerosis（CAS）.Methods A total 0f 62 patients with T2DM were recruited to our study.The intima-media thickness（IMT） and plaques of carotid artery were measured.These patients were divided into 3 groups according to their IMT values：diabetes mellitus（DM） group（n=21）,carotid artery intima thicken group（n=21） and carotid artery intima plaque group（n=20）.At the sane time,20 healthy individuals were selected as control.Serum level of MMP-2,TIMP-1 and hs-CRP were determined and analyzed.Results Compared with the normal control group,the serum levels of MMP-2,hs-CRP were elevated,while TIMP-1 was reduced,the difference was statistically significant（P0.05 or P0.01）;compared with DM group,the serum levels of MMP-2,hs-CRP in intima thicken group were elevated,while TIMP-1 was reduced,the difference was statistically significant（P0.05）;the serum levels of MMP-2,hs-CRP was higher in plaque group than in DM group and intima thicken group,the serum levels of TIMP-1 was lower in plaque group than in DM group and intima thicken group,the difference was statistically significant（P0.05 or P0.01）.Conclusion The severity of carotid atherosclerosis inT2DM is closely associated with the serum MMP-2,hs-CRP level and TIMP-1.
