摘 要:目的:研究雾化吸入肝素对慢性阻塞性肺疾病(COPD)模型大鼠IL-8、TNF-α水平的影响。方法:将大鼠随机均分为对照组、模型(生理盐水)组和肝素高、低剂量(12500、6250u·4mL-1)组,每组10只,后3组气管注入脂多糖并熏香烟以建立COPD模型,各组第8天起雾化吸入相应药物,连续3周。放射免疫法测定血清和支气管肺泡灌洗液(BALF)中IL-8和TNF-α的水平,收集血液和BALF进行白细胞和中性粒细胞计数,观察各组大鼠肺组织切片形态学变化。结果:与对照组比较,模型组大鼠肺泡结构紊乱、肺泡壁变薄断裂,模型组、肝素低剂量组血清和BALF中IL-8及TNF-α水平、白细胞总数、中性粒细胞比例均明显升高(P<0.05或P<0.01);与模型组比较,肝素高剂量组肺组织形态学变化明显好转,血清和BALF中IL-8及TNF-α水平、白细胞总数、中性粒细胞比例明显降低(P<0.05),肝素低剂量组上述指标相比均无明显差异。结论:雾化吸入肝素可能通过限制炎症细胞的活化及组织破坏修复、抑制内皮细胞与白细胞的强黏附过程,发挥减轻COPD炎症的作用。OBJECTIVE:To study the influence of aerosol inhalation of heparin on IL-8 and TNF-α levels in chronic obstructive pulmonary disease(COPD)model rats.METHODS:Rats were randomly divided into control group,model group(normal saline),high-dose and low-dose heparin groups(12 500,6 250 u·4 mL-1)with 10 rats in each group.Latter 3 groups were given lipopolysaccharide and exposured to cigarette to establish COPD model,and aerosol inhalation of relevant medicine were carried out since 8th day in 4 groups for consecutive 3 weeks.Morphology changes of tissue section of lung were observed in each group.Total WBC counts and nertrophil counts in blood and BALF were examined.The levels of IL-8 and TNF-α in serum and BALF were measured by radioimmunoassay.RESULTS:Compared with control group,alveoli disorder of rats was detected and alveolar wall became progressively thinner to break in model group;the levels of IL-8 and TNF-α,total WBC counts and neutrophil counts in serum and BALF in model group and low-dose heparin group were increased significantly(P0.05 or P0.01).Compared with model group,morphology changes of lung tissue in high-dose heparin group were improved significantly;IL-8 and TNF-α levels,total WBC counts and neutrophil counts in serum and BALF decreased significantly(P0.05),and the above index of low-dose heparin group had no significant difference.CONCLUSION:Aerosol inhalation of heparin can reduce COPD inflammation by restricting the activation of inflammatory cells and repair of destroyed tissue,and inhibiting adhesion process of endothelial cell with WBC.
