多排型螺旋CT在胰头癌诊断中应用价值分析

摘  要:目的分析胰头癌的影像学特点并探讨多排型螺旋cT双期扫描在胰头癌诊断中的应用价值。方法对我院2010年1月-2012年6月收治的47例胰头癌患者的影像学资料进行回顾性分析,所有患者取仰卧位,采用双排螺旋CT仪进行平扫与动脉期、门静脉期增强扫描,观察并分析胰头癌的CT影像表现特点。结果胰头癌患者可见胰腺头部增大;CT平扫肿瘤呈等密度或低密度;动脉期增强扫描显示肿瘤出现不均匀环状强化,门静脉期病灶呈密度相对较低,边缘较模糊。患者存在不同程度的肝内胆管扩张、主胰管扩张及胆裳增大。部分患者周围血管受累并有淋巴结转移。结论螺旋CT扫描具有扫描速度快的优点,当注射可以在注射非离子造影剂后,在合适的时间对动脉期及门静脉期进行胰腺容积扫描与薄层扫描,可以准确、清晰地反映胰腺组织情况,对胰头癌的诊断提供客观、可靠的影像学依据。Objective To analyze imaging characteristics of carcinoma of head of pancreas and explore application value of Multi-Detector Spiral CT in diagnosis of the carcinoma. Methods Imaging data of forty seven patients with carcinoma of head of pancreas treated in our hospital from Jan. 2010 to Jun. 2012 were put under regressive analysis. All the patients at supine position received plain MDCT scanning, arterial phase and portal venous phase enhancement scanning to observe and analyze the CT imaging characteristics. Results Enlargement of pancreas head was found in those patients; tumors showed the same or low density by plain CT scanning; arterial phase enhancement scanning indicated tumors had unevenly ring shape enhancement and portal venous phase had a low density of focuses with obscure edge. The patients showed varyingly intrahepatic duct dilatation, main pancreatic duct dilatation and .gallbladder enlargement. Some of the patients suffered from peripheral vessels involvement and lymphatic metastasis. Conclusion The spiral CT features its fast speed. If injection can be made after injecting non-ionic contrast agent, arterial phase and portal venous phase are duly scanned in terms of pancreatic volume and thin layer to accurately and clearly reflect pancreatic tissues, offering an objective, reliable basis for diagnosis of carcinoma of head of pancreas.
