摘  要:目的探讨实时全容积心脏成像评价早期新生儿心功能的价值。方法应用实时全容积心脏成像动态监测新生儿第1~4天的左、右室功能的变化,包括舒张末期容积、收缩末期容积、每搏输出量及射血分数。其测量左室功能指标与M型超声心动图测量值进行对比研究。结果实时全容积心脏成像能实时、快速地在1个心动周期内同时测量左、右室功能;线性回归分析表明实时全容积心脏成像测量的左室舒张、收缩末期容积,每搏输出量及射血分数与M型超声心动图测量值之间具有较好的相关性(r=0.51、0.45、0.48、0.61,P均为0.000)。早期新生儿(第1~4天)的右室舒张、收缩末期容积及每搏输出量均明显高于左室(P<0.05),但右室射血分数要明显低于左室(P<0.05)。另外,第1~4天的新生儿左室各测量指标之间、右室各测量指标之间的差异无统计学意义(P>0.05)。结论实时全容积心脏成像能准确、同步地评价早期新生儿左、右室功能。Objective To evaluate the value of ventricular function in early neonatal period by real-time full-vol ume cardiac imaging. Methods Dynamic change of left and right ventricular funtion was obtained by real-time full-vol ume cardiac imaging in neonates from the Ist day to 4th day, the indexes included end-diastolic volume, end systolic vol ume, stroke volume and ejection fraction. And the results of left ventricular funtion were compared with those of M-mode echocardiography. Results Real-time full-volume cardiac imaging could determine the left and right ventricu- lar function in a cardiac cycle on-line rapidly. The simple regression analysis demonstrated the measurement of left ventricular functon indexes were excellent correlation between the real-time full-volume cardiac imaging and the M-mode echocardiography. The right ventricular enddiastolic volume, end systolic volume and stroke volume were greater than those of the left ventricle in early neonatal period, but the left ventricular ejection fraction was higher than that of the right ventricle. At the same time, there were no differences in the end-diastolic volume, end systolic vol- ume, stroke volume and ejection fraction in neonates from the Ist day to 4th day. Conclusions Real-time full-volume cardiac imaging can accurately quantify left and right ventricular function in a cadiac cycle in early neonatal period.
