摘  要:目的探讨断层超声显像(tomographic ultrasound imaging,TUI)联合时空间相关成像(spatio-temporal image correlation,STIC),即TUI-STIC技术,诊断胎儿圆锥动脉干畸形(conotruncal defects,CTD)的可行性。方法回顾分析167例CTD胎儿超声资料,对比二维超声心动图(two dimensional echocardiography,2D-Echo)、TUI-STIC方法对胎儿CTD的诊断准确率、扫描时间,应用TUI-STIC技术分析各类胎儿CTD的声像学特点。结果与2D-Echo相比,TUI-STIC诊断胎儿CTD准确率高(P<0.05),扫描时间少(P<0.0001);TUI-STIC分析胎儿CTD声像学特点为心室与大动脉连接关系异常、连续性中断、大动脉空间关系异常、血管数目异常等。结论与2D-Echo相比,TUI-STIC诊断胎儿CTD准确率高,扫描时间少,可提供与CTD病理解剖相关的信息。Objective To explore the feasibility of tomographic ultrasound imaging (TUI)with spatio-temporal image correlation(STIC) in prenatal diagnosis of fetal conotruncal defects(CTD). Methods Medical records of one hundred sixty-seven fetuses with CTD were reviewed. Two-dimensional echocardiograph(2D-Echo)and TUI-STIC were used to display the four-chamber view, the left and right outlet tract view and three-vessel view respectively. The diagnose accordance rate and screening time by two methods were compared and the sonographic characteristic of CTD by TUI STIC were analyzed. Results The diagnose accordance rate of fetal CTD by TUI-STIC was higher than that by 2D-Echo (P〈 0.05). The screening time with TUI-STIC was shorter than that by 2D-Echo(P〈 0. 0001). The ab normal features of CTD were interruption of septoaortic continuity, abnormal relationship between aorta and pulmona ry artery and abnormal number of vessel. Conclusions By contrast to 2I)-Echo, the diagnose accordance rate of fetal CTD by TUI-STIC was higher and the screening time with TUI-STIC was shorter. TUI-STIC can give important in formation about morphological characteristic of CTD.
