摘  要:目的探讨应变率成像技术评价2型糖尿病患者左室局部功能早期损害的临床价值。方法选取左室构型正常的2型糖尿病患者(糖尿病组)20例,健康者20例为对照组。应用常规超声心动图测量二尖瓣口舒张期血流E峰、A峰,计算E/A,应变率成像技术测量左室长轴方向18节段的心肌收缩期峰值应变率(SRs)、舒张早期峰值应变率(SRe)和舒张晚期峰值应变率(SRa),并计算SRe/SRa,比较两组各参数的差异。结果糖尿病组E峰、E/A低于对照组,A峰高于对照组,糖尿病组各节段室壁运动收缩期SRs低于对照组,糖尿病组舒张期SRe、SRa和SRe/SRa均低于对照组(均P<0.05)。结论应变率成像技术可以定量评价糖尿病患者的左室局部心肌应变率异常,SRs可以作为评价左室局部收缩功能的有效指标,SRe、SRa和SRe/SRa可以作为评价左室局部舒张功能的有效指标。Objective To explore the clinic value of strain rate imaging in assessing left ventricular regional systolic and diastolic function in patients with type 2 diabetes mellitus. Methods Twenty patients with type 2 diabetes and 20 healthy volunteers were enrolled. Peak velocity of E and A were measured by conventional echocardiography, and E/A was calculated. The peak systolic strain rate (SRs), early diastolic (SRe) and late diastolic (SRa) strain rate were measured by strain rate imaging in left ventricular long-axis view, and SRe/SRa was calculated. The parameters between the two groups were compared. Results E and E/A were lower in diabetes mellitus group than in control group. A were higher in diabetes mellitus group than in control group. SRs, SRe, SRa and SRe/SRa were lower in diabetes mellitus group than in control group (P 〈 0.05). Conclusion Strain rate imaging technology can quantitatively evaluate left ventricular regional myocardial strain rate abnormalities in patients with diabetes mellitus. SRs can be used as an effective indicator in evaluating left ventricular regional systolic function. SRe, SRa and SRe/SRa can be used as effective indicators in evaluating left ventricular regional diastolic function.
