高血压患者的左心腔内径变化及心房颤动发生率的分析

摘　　要：目的:左心腔内径的增大是心血管疾病和心房颤动的重要危险因子,左心腔内径的大小与高血压患病时间的相关性有待论证。方法:选择本社区高血压人群2000例,非高血压人群500例,符合高血压标准的为1605例,男性671例,女性934例,符合非高血压组标准的人群为432例,男性85例,女性347例,应用超声心动图测量左心腔内径来调研比较高血压患者和正常人群的差异,高血压伴有房颤、患高血压病的时间、抗高血压治疗时间与左心腔内径变化的关系。结果:我们的基线调研显示,高血压患者左心腔内径(包括左心房与左心室)较非高血压者增大,高血压患者心房颤动发生率(5.9%)明显高于非高血压者(2.8%),P<0.05。结论:高血压患者左心腔内径增大,其心房颤动发生率明显增加,社区干预高血压可降低左心腔内径的扩大和心房颤动的发生。Objective： The left cardial diameter increase is the important risk factor of atrial fibrillation （AF） and cardiovascular disease. The relationship of the size of the left cardial diameter with the occurrence time of hypertension will be demonstrated. Method： A population of 2000 community hypertension and 500 non-hypertension people were chosen, in which 1 605 cases met the standard of hypertension, and 671 were male and 934 female. Four hundred and thirty two cases of the non- hypertension group conformed the non-hypertensive group standard, and 85 cases were male, 347 female. Echocardiography measuring the left ventricular cavity diameter was applied to compare the differences between the hypertension group and the normal population, and the correlations of hypertension accompanied with atrial fibrillation, times of suffering from hypertension and anti-hypertensive treatment with the change of the left ventricular cavity diameter. Result： The baseline survey showed that the left ventricular cavity diameter increase was obvious （including left atrium and left ventricular）. The incidence of the atrial fibrillation of the hypertension patients was obvious higher （5.9%） than that of non-hypertension patients （2.8%,） （P〈0.05）. Conclusions： The size of the left cardial diameter of the hypertension patients increases, whose atrial fibrillation incidence obviously increases. Community intervention of hypertension can reduce the expansion of the left cardial diameter and the incidence of atrial fibrillation.
