高血压早期患者BPV和踝-臂指数的年龄差异

摘　　要：目的探讨高血压早期患者血压变异率(BPV)与动脉弹性踝-臂指数(ABI)的年龄差异。方法未经服药治疗的高血压早期患者68例分为两组:A组41例,年龄<60岁;B组27例,年龄≥60岁。依据24-h动态血压测定不同时间多次血压的脉压(PP)及其均值标准差(SD)来确定BPV;同时采用全自动动脉弹性检测仪检测ABI。结果 B组24-h平均脉压、日间平均脉压、夜间平均脉压和夜间收缩压差值SD均高于A组[(63.4±17.4)mmHg vs.(51.4±6.0)mmHg、(63.6±17.0)mmHg vs.(51.7±6.2)mmHg、(61.4±21.0)mmHg vs.(49.4±8.0)mmHg和(11.2±3.7)mm Hg vs.(8.9±3.3)mm Hg](P<0.05)。B组ABI异常发生率也高于A组(51.9%vs.24.4%)(P<0.05)。结论≥60岁高血压早期患者的24-h平均脉压、日间平均脉压、夜间平均脉压、夜间收缩压差值及ABI异常率均高于<60岁者。Objective To investigate the age-difference of blood pressure variability（BPV） and ankle-brachial index（ABI） in patients with essential hypertension（EH） in the early stage.Methods A total of 68 patients with EH in the early stage,who did not take antihypotensive drugs,was divided into two groups of A（aged 60 years,41 cases） and B（aged ≥60 years,27 cases）.The BPV was determined by mean pulse pressure（PP） recorded in 24 hours and its standard deviation（SD）.Ambulatory blood pressures in 24 hours were recorded and ABI was measured by Colin system.Results The mean 24-h PP,mean daytime PP,mean night PP and SD value of PP difference from night SBP were all higher in group B than those in group A [（63.4±17.4） mm Hg vs.（51.4±6.0） mm Hg,（63.6±17.0） mm Hg vs.（51.7±6.2） mm Hg,（61.4±21.0） mm Hg vs.（49.4±8.0） mm Hg and（11.2±3.7） mm Hg vs.（8.9±3.3） mm Hg]（P0.05）.The incidence rate of abnormal ABI was higher in group B than that in group A（51.9% vs.24.4%）（P0.05）.Conclusion The mean 24-h PP,mean daytime PP,mean night PP,SD value of PP difference from night SBP and the incidence rate of abnormal ABI are all higher in the early stage EH patients aged ≥60 years old than those 60 years.
