摘 要:目的 探讨间接免疫荧光法(IIF)测定尖锐湿疣(CA)疣体表面细胞中人类乳头瘤病毒的敏感性和特异性,及其在尖锐湿疣早期诊断中应用的可行性。方法 收集CA标本52例,对照组50例,分别用刮片法(刮取疣体表面细胞法)及涂片法(切除疣体涂片)取材,用间接免疫荧光法检测。所有标本均进行醋酸白试验、组织病理诊断和PCR法检测。结果 刮片法取材CA组和对照组阳性率分别为51.92%和0。涂片法取材分别为76.92%和0。提示在CA的检测中用刮片法取材的敏感性和特异性分别为51.92%和100%;涂片法的敏感性和特异性分别为76.92%和100%。刮片法的敏感性低于涂片法,差异有统计学意义(P<0.01)。PCR法检测示CA中HPV-6阳性率53.85%和HPV-11型为42.31%。结论 用间接免疫荧光法检测HPV时用表面刮取细胞法取材的敏感性不高,但特异性强,可以作为诊断CA的辅助手段,可提高人类乳头瘤病毒的早期检出率。PCR法检测提示HPV-6和11型为最常见。Objective To study the sensitivity and specificity of the indirect immunofluorescence(IIF) technique in de- tection of the human papillomavirus in eondyloma acuminatum (CA) surface cells, to explore the feasibility of IIF assay in early diagnosis of CA. Methods Collected 52 cases of CA specimens,and 50 cases of non-CA as controls, using the scraping method (scraping the wart surface cells) and the smearing method (removal of the wart and smearing) to detect IIF assays. All specimens utilized acetic acid white test, the histopatho- logic diagnosis and the PCR assay. Results The positive rate in CA group and control with scraping method were 51.92%, 0 respectively; and the smearing method were 76.92%, 0 respectively. The resuhs showed the sensitivity and specificity by IIF detection of CA with scraping method were 51.92% and 100% ; with smearing method were 76.92% and 100%. The sensitivity of the scraping method was lower than the smea- ring, there were statistically significant differences(P 〈 0.01 ). PCR assay showed that the HPV-6 and HPV- 11 positive rate in CA were 53.85 % and 42.31% respectively. Conclusion IIF assay to HPV detection in scraping cells from the surface of the CA is a method with moderate sensitivity and high specificity. It can be used as an adjunct method in CA diagnosis, and to improve early detection rates of human papillomavirus. PCR assay showed HPV-6 and 11 were the most common types.
