多发性硬下疳合并二期梅毒1例

摘　　要：患者男,34岁。阴茎、阴囊反复起溃疡6个月,躯干、大腿起红斑5d。查体:阴茎及阴囊可见多处浅溃疡,直径0.5～1.0cm,界清,边缘整齐,呈软骨样硬度,无触痛。躯干、大腿见广泛分布类圆形淡红斑,大小不一;双手足未见红斑和脱屑。实验室检查:TPPA(+),RPR1:128(+),HIV抗体阴性。HSV-IIgG(+),HSV-IIIgG(+),HSV-IIgM(-),HSV-IIIgM(-)。诊断:多发性硬下疳;二期梅毒。予苄星青霉素240万U分两侧臀部肌注,1次/周,连用3次后皮损消退。A 34 years men, with painless ulcers on his penis and scrotum repeatedly for six months, and with erythema on the trunk and thigh for 5 days. Examination ： there were many well-demarcated, painless ulcers on the the penis and scrotum,0.5 to 1.0cm sized, which touched as cartilage-like papules. Many red round erythemas on the trunk and thigh, and no erythema and desquamation on hands and feet. Laboratory tests ： TPPA （＋） , RPRl：128 （＋） , HIV （ -）,HSV-Ⅰ IgG（ ＋）,HSV-Ⅱ IgG（ ＋）,HSV-Ⅰ IgM（ -）,HSV-Ⅱ IgM （ - ）. Diagnosis ; secondary syphilis with multi-chancre. Benzathine penicillin G 2.4 MU injections were given to each buttock once a week for 3 weeks, then chancres and syphilitic macules disappear.
