多层螺旋CT在复杂食源性异物术前评估中的意义

摘　　要：目的探讨多层螺旋CT(multislice spiral CT,MSCT)在复杂食源性异物术前评估中的价值。方法对24例术后证实为食源性异物患者进行术前评估,单独或联合应用X线摄片、吞服造影剂摄片、电子内镜检查、MSCT扫描4种检查方法,评估异物的形状及空间定位、与主动脉的关系、是否伴并发症。结果应用前3种检查方法诊断异物20例(83.3%),满意完成上述术前评估6例(25%),进一步应用MSCT扫描及其后处理技术,全部诊断异物并满意完成上述术前评估(100%)。结论多层螺旋CT结合其后处理技术在复杂食源性异物,尤其是与主动脉关系密切的不规则异物及游走至消化道外的异物的术前评估中具有极高的实用价值,值得首选。Objective To study the values of multislice spiral CT in preoperative assessment of complicated foodborne foreign bodies.Method 24 patients with foodborne foreign bodies proved by surgical were examinated preoperation by the following 4 methods： X-ray after take contrast medium or not,electronic-endoscopes,MSCT with 3D reconstruction,then assessed the shape,location,relationship with aorta,and complications of foreign bodies.Result 83.3%（20/24） cases were detected foreign bodies,and 25%（6/24） cases were assessed preoperation satisfactory by the first three methods,nevertheless,their all were detected and assessed satisfactory when use MSCT with 3D reconstruction.Conclusion MSCT with 3D reconstruction has practical values in preoperative assessment of complicated foodborne foreign bodies,especially the foreign bodies close to aorta with irregular shape,or migrate out of the digestive tracts,which could be a preferred examination.
