中国商环包皮环切术两种术式临床比较

摘　　要：目的对比内环置于包皮外板法与内环置于包皮腔内法中国商环包皮环切术的临床疗效及手术并发症。方法按单双号分组，将患者分为内环置于包皮外板法组（内环外板法组）与内环置于包皮腔内法组（内环腔内法组），每组25例。对比分析2组在手术时间、术时及术后24h疼痛评分、失血量、创口愈合时间、术后并发症、术后外观满意率等方面的差别。结果2组手术时间、术时及术后24h疼痛评分、失血量比较差异无统计学意义。但内环外板法组较内环腔内法组术后感染率（4％ vs．32％，χ2=4．88，P〈0．05）及水肿发生率（0vs．24％，χ2=4．73，P〈0．05）明显降低、创口愈合时间明显缩短[（12．60±1．58）d vs．（14．28±2．84）d，t=2．59，P〈0．05]、外观满意率明显增高（100％VS．76％，）（χ2=4．73，P〈0．05）。结论内环置于包皮外板法比内环置于包皮腔内法具有操作简单、并发症发生率低、术后愈合时间短、外观满意率高等优点。Objective To compare the clinical efficacy and postoperative complications of two methods of Chinese ShangRing circumcision. Methods 50 patients were divided into two groups by the odd and even outpatient No. , 25 pa- tients in each group. Outside of the foreskin group is that the inner ring of the Chinese ShangRing is placed on the outside of the foreskin, inside of the foreskin group is that the inner ring of the Chinese ShangRing is placed in the inside of the foreskin. Operation time, pain score during the operation and 24 h postoperation, blood loss, wound healing time, postop- erative complication, and postoperative appearance were compared. Results In the operation time, the pain score during the operation and 24 h postoperation, the blood loss, the difference between the two groups were not statistically significant. The postoperative infection and edema of the outside of the foreskin group were significantly lower （4% vs. 32% , χ2 = 4.88, P 〈 0.05 ; 0 vs. 24% , χ2 = 4. 73, P 〈 0.05 ）, the wound healing time of the outside of the foreskin group was signif- icantly decreased[ （ 12.60 ± 1.58） d vs. （ 14.28 ± 2.84） d, t = 2. 59,P 〈 0.05 ], and the satisfaction rate of postoperative appearance of the outside of the foreskin group was significantly increased （ 100% vs. 76% , χ2 = 4.73, P 〈 0.05 ） , than the inside of the foreskin group. Conclusions Compare with the inner ring of the Chinese ShangRing is placed in the in- side of the foreskin method, the advantage of the inner ring of the Chinese ShangRing is placed on the outside of the fore- skin Method are simple operation, low complication rate, post operative healing time is short, the appearance satisfaction rate is high.
