输尿管镜钬激光肾盂旁囊肿内引流术（附26例报告）

摘　　要：目的探讨输尿管镜钬激光切开内引流术治疗肾盂旁囊肿的手术方法、疗效和安全性。方法自2005年9月-2011年11月收治的26例患者，共29侧挤压肾盂或肾盏且皮质较厚的肾盂旁囊肿，其直径5～9cm，腰腹部不适9例，血尿5例，局部肾盏积水3例，合并高血压3例，泌尿系感染3例，合并结石2例。行输尿管镜钬激光肾盂旁囊肿内引流术，收集病例的临床资料。结果其中24例患者成功，手术时间（20～50）min，未发现尿外渗及其他明显并发症，2例手术中无法明确囊肿位置，改行经皮囊肿穿刺术。通过复查IVP、B超或CT，随访（6～18）个月，平均11．5个月。治愈19侧，有效8侧，囊腔均与肾盂相通。结论对此类型肾盂旁囊肿，行输尿管镜钬激光内引流术操作简单，创伤小、安全、有效。Objectives To discuss the surgical approaches,efficacy and safety of Holmium laser uretero- scopic intrapelvic drainage in the treatment of parapelvic cyst. Methods 26 patients（29 sides）were brought into the investigation since September 2005 to November 2011. The parapelvic cyst had squeezed the renal pelvis and had a thick cortex. The diameter （5 - 9 ） era,waist and abdomen discomfort 9 cases, hematuria 5 cases, hydronephrosis 3 cases, hypertension 3 cases, urinary tract infection 3 cases, 2 cases with stones. By Holmium laser parapelvic cyst drainage, we collected the clinical information. Results 24 cases had succeed. Operative time （20 - 50） min, urine extravasation and surgical complications were not found. 2 cases which could not clear cyst location failed, then diverted to percutaneous cyst puncture. Review of Intravenous pyelogram（ IVP）, B ultrasound or CT, follow- up （6 - 18） months showed 19 sides cured, 8 sides improved and the cavities were connected with the pelvis. Conclu- sions For this type of parapelvic cyst, holmium ： YAG laser ureteroscopic intrapelvic drainage is a simple, mini- really invasive, safe and effective method.
