大承气汤对大鼠急性重症胰腺炎并发腹腔高压的影响

摘　　要：目的观察中药大承气汤对大鼠急性重症胰腺炎(SAP)并发腹腔高压的影响。方法选用健康雄性SD大鼠60只,随机分为三组,假手术组、SAP组、大承气汤治疗组,每组20只。通过腹腔置管直接测定大鼠急性重症胰腺炎时腹腔内压力。检测2周后实验动物死亡率、胰腺病理学和腹水细菌学、门静脉血内毒素(ET)和体静脉血淀粉酶。结果 SAP组、大承气汤治疗组间动物死亡率、血清淀粉酶水平差异无统计学意义(P>0.05),胰腺病理改变类型及程度相似;大承气汤治疗组腹腔内压力显著低于SAP组(P<0.05),门静脉ET水平及腹水细菌阳性率亦显著低于SAP组(P<0.01及P<0.05)。提示大承气汤可有效减轻SAP大鼠腹腔内压力,减少腹腔感染发生率。结论大承气汤可能在一定程度上减轻大鼠急性重症胰腺炎并发腹腔高压。Objective To study the effect of Dachengqi Decoction on intra-abdominal hypertension complicating with severe acute severe acute pancreatitis. Methods 60 healthy male SD rats were randomly divided into three groups, the sham group, SAP group, Dachengqi Decoction treatment group, each group of 20 only. Peritoneal catheter directly used for determi- nation of the pressure within the abdominal cavity of rats with severe acute pancreatitis. After two weeks we could detect the exper- imental animals mortality, pancreatic pathology and ascites bacteriology, portal blood endotoxin （ET） and body venous blood amylase. Results Between SAP group and treatment of Dachengqitang group, the differences in animal mortality and in ser- um amylase level were no statistically significant （P 〉 0. 05 ）, the type and extent of the pathological changes of the pancreas is similar. The pressure in the abdominal cavity of the Dachengqi Decoction treatment group was significantly lower than the SAP group （P 〈 0. 05）, portal venous ET levels and ascites bacterial positive rate was also significantly lower than the SAP group （ P 〈0.01, P 〈0.05） . The prompt Dachengqi Decoction group maybe reduce SAP rats intra-abdominal pressure and abdominal infection incidence. Conclusion Dachengqi Decoction maybe relieve abdominal hypertension in rats with severe acute pancre- atitis complicated.
