摘 要:目的观察不同输液量对腹腔感染行机械通气治疗患者肺顺应性、氧合指数及痰液黏稠度的影响。方法选取ICU 100例腹腔感染行机械通气的患者,于机械通气治疗第2天开始,根据24h液体治疗计划输入晶体液总量的不同分为3组,A组〈2500ml/24h:B组2500-3500ml/24h;C组〉3500ml/24h,分析3组在输液后8h、16h和24h的肺顺应性、氧合指数、痰液黏稠度及吸痰频率。结果输液后8h、16h、24h,A组肺顺应性优于B组和C组(P〈0.001)。输液后8hA组和B组氧合指数明显高于C组,而在16h、24h后A组氧合指数高于B组和C组(P〈0.05)。输液后8h、16h、24h,B组和C组Ⅰ度黏稠痰液的发生率高于A组(P〈0.05),吸痰频率高于A组。结论针对腹腔感染行机械通气治疗患者,输入晶体液的总量〈2500m1/24h对改善患者预后是比较有效的选择。Objective To investigate the impact of different infusion volume on the lung compliance,oxygenation index and sputum viscosity during mechanical ventilation in patients with intra-abdominal infection. Methods One hundred patients with intra-abdominal infection receiving mechanical ventilation in the ICU were recruited in this study. They were divided into 3 groups based on the infusion volume of crystal liquids within 24 hours:less than 2500ml in group A,2500-3500ml in group B and more than 3500ml in group C. Analysis the parameters of 8h,16h and 24h in patients with lung compliance,oxygenation index, sputum viscosity,and suctioning frequency in each groups. Results The lung compliance in group A was significantly higher than that of group B and group C(P〈0.001 ) at 8h, 16h,and 24h after fluid infusion. The oxygenation index at 8h after fluid infusion was significantly higher in group A and group B compared to group C(P〈0.05),which at 16h and 24h was significantly higher in group A than that of group B and group C(P〈0.05). The incidence of I-sputum viscosity and suctioning frequency was lower at 8h,16h and 24h after fluid infusion in group A compared to group B and group C(P〈0.05). Conclusion The infusion volume less than 2500ml within 24h is more effective to improve the prognosis for patients with intra-abdominal infection receiv- ing mechanical ventilation.
