肿瘤化疗药物配制中常见问题分析与对策

摘　　要：目的分析肿瘤化疗药物配制过程中常见问题出现的原因并提出整改措施,保证临床用药的安全。方法对2009～2011年我院肿瘤化疗药物配制中心出现的3180例次问题进行回顾性分析。结果肿瘤化疗药物配制中出现的问题主要发生在医嘱录入、药师审方排药、护士配药、成品核对4个主要环节。结论可通过规范规章制度、强化思想教育、加强业务培训、使用现代化管理仪器、护士配药环节质量监控等措施,减少常见问题的发生,提高肿瘤化疗药物的配制安全性。Objective To analyze the causes and countermeasures of the problems in dispensing chemotherapeutics for cancer patients,and to ensure the safety of medication administration. Methods The problems were analyzed based on the data of 3180 cases in the chemotherapeutics admixture centre from 2009 to 2011 in our hospital. Results The problems in dispensing chemotherapeutics mainly involved in the process of physician orders entry,prescription check by pharmacists,pharmacy admixture by nurses and check after admixture. Conclusion It is suggested to standardize the rules and regulations, strengthening nurse training,utilizing modem management instruments and enforce quality control of pharmacy admixture to reduce the problems in dispensing chemotherapeutics and ensure the safety of medication administration.
