糖尿病肾病大鼠nephrin的表达及通络泄浊方的干预作用

摘　　要：目的研究nephrin在糖尿病肾病(DN)大鼠肾组织中表达的变化,并探讨通络泄浊方对糖尿病大鼠肾组织nephrin表达的影响。方法采用链脲菌素STZ法制作大鼠模型,随机分为正常对照组、模型组、通络泄浊方组(中药组)、氯沙坦组(西药组)和通络泄浊方联合氯沙坦组(中西药组)。各组采取相应的干预措施。12周时检测大鼠尿微量白蛋白排泄率(UAER)、血肌酐(Scr)、尿素氮(BUN)、肾重指数(K/Bw);采用HE、PAS染色法及透射电镜观察大鼠肾组织病理情况;采用免疫组化、Real-time PCR及Westernblot方法检测DN大鼠肾组织nephrin mRNA和蛋白的表达。结果与正常组比较,模型组尿微量白蛋白排泄率、血肌酐、尿素氮及肾重指数均明显升高,而nephrin mRNA表达和蛋白均明显降低(P<0.01);与模型组比较,各给药组尿微量白蛋白排泄率、血肌酐、尿素氮及肾重指数均有不同程度的降低,nephrin mRNA和蛋白表达均明显增高(P<0.05)。结论 Nephrin表达降低参与了糖尿病肾病大鼠蛋白尿的发生;通络泄浊方通过上调DN大鼠肾组织nephrin mRNA和蛋白的表达而改善大鼠的肾功能及蛋白尿情况。Objective To investigate the nephrin expression in diabetic rats and its effects by ＂Tongluo Xiezhuo Decoction＂.Methods Streptozotocin（STZ）-induced diabetic rats were established,and the rats were randomized into five groups： normal group,model group,＂Tongluo Xiezhuo Decoction＂ group,losartan group and ＂Tongluo Xiezhuo Decoction＂ plus losartan group.At the end of 12 weeks,the 24 h urinary albumin excretion rate（UAER）,blood urea nitrogen（BUN） and serum creatinine（Scr） were measured.The kidney tissues were dealt by HE staining and PAS staining to observe the pathologic changes under electronic microscope.Real-time PCR,immunohistochemistry and Western blot were employed to study the change of nephrin mRNA and protein.Results Compared with the normal group,UAER,BUN and Scr were higher and the expression of nephrin mRNA and protein was lower in the model group（P0.01,P0.05）.Compared with the model group,UAER,BUN and Scr were lower and the expression of nephrin mRNA and protein was higher in three treatment groups（P0.01,P0.05）.Conclusion Nephrin expression is associated with diabetic nephropathy and ＂Tongluo Xiezhuo Decoction＂ can up-regulate nephrin expression to reduce urinal protein in diabetic nephropathy rats.
