个案管理护理改善创伤后截瘫患者康复期生存质量的效果

摘　　要：目的探讨个案管理护理改善创伤后截瘫患者康复期生存质量的效果。方法将80例创伤后截瘫患者按病区分为研究组和对照组各40例,研究组接受为期6个月的个案管理护理,对照组接受常规护理。分别比较两组患者干预前及干预后1、3、6个月的生存质量、日常生活活动能力及康复依从性。结果干预后1、3、6个月,研究组在生存质量总分、生理维度、心理维度、与医务人员关系维度等方面的得分均高于对照组,差异均有统计学意义(P<0.01);干预后3个月和6个月时,研究组日常生活活动能力得分均低于对照组,差异有统计学意义(P<0.05或P<0.01);干预后1、3、6个月,研究组患者康复依从性均优于对照组,且研究组干预后3个月和6个月时的依从性也优于干预后1个月,差异均有统计学意义(P<0.05)。结论对康复期的创伤后截瘫患者开展个案管理护理能够改善其生存质量,提升患者康复的依从性和日常生活能力,提高康复效果。Objective To explore the nursing case management practice in improving the quality of life （QOL） in patients with traumatic paraplegia. Methods Patients with traumatic paraplegia were divided into experimental group and control group according to the ward location, with 40 cases in each group. 6 months of nursing case management was implemented in the experimental group, and the routine nurs- ing was implemented for the control group. Comparisons were conducted on the QOL,activity of daily liv- ing and compliance of rehabilitation before and 1, 3 and 6 months after the intervention. Results Com- pared with the control group, the total score and the scores of psychological dimension, physical dimension and dimension of medical personnel relationship in the experimental group were significantly higher at 1, 3 and 6 months after the intervention（P〈0.01）; the score of activity of daily living in the experimental group was significantly lower at 3 and 6 months after the intervention（P〈0.05 or P〈0.01） ; the compli- ance of rehabilitation in the experimental group was significantly improved at 1,3 and 6 months after the intervention; the compliance of rehabilitation in the experimental group at 3 and 6 months after the inter- vention was significantly superior to that at 1 month after the intervention（P〈0.05）. Conclusion Nursing case management program can improve the QOL in patients with paraplegia,elevate the compliance of re- habilitation and the activity of daily living and enhance the effect of rehabilitation.
