氨力农治疗难治性心力衰竭对照研究

摘　　要：目的　观察氨力农治疗难治性心力衰竭病人心功能变化和临床疗效。方法　治疗组 30例难治性心衰患者加用氨力农 ,先静注 5 0mg(2 0min) ,再静滴 15 0mg(36 0min)。对照组 18例则改用生理盐水。测定治疗前 ,治疗后 2 0min和 36 0min时心率和心功能 (SV、CO、CI) ,治疗组还测定一疗程后心功能 ,并作临床疗效评估。结果　治疗组SV、CO、CI明显增高 ,最大效应出现在静注负荷量后 ,继续静脉滴注可维持疗效 ,但不再增加作用 ,一疗程后 79%病人临床心功能有 1～ 2级改善。对照组心功能无变化。结论　对难治性心衰患者 ,氨力农是合理而有效的选择之一。用氨力农治疗心衰时先静脉注射负荷量有利于迅速取得疗效 ,并可通过静滴维持量保持疗效。Objective To observe the change of heart function and clinical curative effect of refractory heart failure patient treated by amrinone Methods The therapeutic group has 30 patients with heart failure.Amrinone was used First 50mg was injected in 20 min,then 150mg infused by intravenous duip in 360 min 18 patients of control group were treated with normal saline. Heart rate and heart function (SV,CO,CI)were individually tested pretreatment,20 and 360min after injection.Heart function was also tested in therapeutic group after one course of treatment,and therapeutic efficiency evaluated Results The index of heart function improved obviously of therapeutic guoup The maximum effect was at that loading dose infused.Therapeutic effect could be maintained by continuous infusion by intravenous drip,but no more effect was observed.After one course of treatment,79% of patients' heart function improved by 1-2 degree,while no change observed in control group Conclusions To the patients with refractory heart failure,amrinone is one of the reasonable and effective choice.Loading dose of amrinone can get the therapeutic effect quickly,which can be maintained by continuous infusion.
