腹腔镜和传统剖腹探查方法治疗非创伤性急腹症的临床比较

摘　　要：目的研究腹腔镜探查在治疗非创伤性急腹症的优越性。方法在研究人员基本固定情况下,将2007年3月至2010年6月间的730例外科与妇科几种非创伤性急腹症患者随机实施腹腔镜探查治疗(简称A组)与传统的剖腹探查治疗(简称B组)观察两组的平均诊断时间、平均手术时间、平均住院时间、术后并发症发生率四个指标,采用配对t检验和x2检验作统计学处理。结果术后发生并发症A组10例,B组22例,无死亡病例。A组平均诊断时间、手术时间、住院时间、术后并发症发生率均明显少于B组(P<0.05)。结论腹腔镜探查治疗非创伤性急腹症较传统剖腹探查治疗的优势明显,能及早准确诊断、快速治疗,并发症少、住院时间短。腹腔镜探查治疗非创伤性急腹症在基层二甲医院安全可行,同样具有推广、应用价值。Objective To observe the advantage of laparoscopy in the treatment of non-traumatic acute abdomi-nal emergency. Methods Between March 2007 and June 2010, 730 patients with surgical or gynecological acute abdomen underwent diagnosis and treatment of laparoscopic exploration （group A） or traditional exploratory laparotomv （group B） randomly. The average diagnostic duration, the average duration of operation and the average hospital stay of the two groups were observed, statistical analysis was done with the paired t test and x2 test. Results There were 10 cases of postoperative complications in group A, and 22 cases in group B, no death occurred. The average diagnostic duration, the average operation duration, hospital stay and postoperative complications in group A were less than group B. Conclusion Laparoscopic exploration has significant advantage over traditional exploratory laparotomv with early and accurate diagnosis, rapid treatment. It results less complications and shorter hospital stay. Laparoscopic explora- tion is safe and applicable in Grade 2A hospital, which is also suitable for wider use in diagnosis and treatment of non- traumatic acute abdominal emerzency.
