出院计划在脑卒中患者照顾者中的应用

摘　　要：目的探讨出院计划在脑卒中患者照顾者中的应用效果。方法将112例照顾者按照随机数字表法随机分为对照组和观察组各56例。对照组照顾者接受常规护理，观察组在常规护理基础上实施出院计划护理干预。评估两组照顾者干预前、干预后的照顾结果和需求满足程度。结果干预前观察组与对照组照顾结果得分分别为（46．51±5．01），（47．28±4．63）分，干预前观察组与对照组需求满足程度得分分别为（1．84±0．52），（1．67±0．63）分，差异均无统计学意义（t值分别为-0．84，1．56；P均〉0．05）。实施出院计划后，观察组照顾者出院后4周的照顾结果为（53．71±3．56）分，优于对照组的（48．49±3．63）分；出院后4周的需求满足程度为（3．55±0．47）分，高于对照组的（2．29±0．79）分，差异均有统计学意义（t值分别为7．68，10．26；P均〈0．05）。结论实施出院计划可以明显改善脑卒中患者照顾者的照顾结果及需求满足程度。Objective To explore the effect of discharge planning on caregivers of stroke patients. Methods A total of 112 patients were randomly divided into control group and intervention group, and each group had 56 patients. The caregivers in control group received routine interventions, while caregivers in intervention group received discharge planning nursing interventions based on routine interventions. The care ability and demand satisfaction of caregivers in two groups pre - intervention and post-intervention were observed. Results Care ability score and demand satisfaction score of intervention group and control group respectively were [ （46.51 ± 5.01 ） vs （47.28 ± 4.63 ） and （ 1.84 ± 0. 52） vs （ 1.67 ± 0. 63 ） ], and the differences were statistically significant （ t = - 0.84,1.56 ; P 〉 0.05 ）. After discharge planning, care ability score of intervention group was significantly higher than that of control group in 4th weeks after discharge [ （53.71 ± 3.56） vs （48.49±3.63 ） ] （ t = 7.68, P 〈 0.05 ）. Demand satisfaction score of intervention group was significantly higher than that of control group [ （ 3.55 ± 0.47 ） vs （ 2.29 ± 0.79） ] （ t = 10.26, P 〈 0. 05 ）. Conclusions The implementation of discharge planning can significantly improve the care ability and need satisfaction of caregivers of stroke patients.
