摘　　要：目的阐述有孔型基底动脉变异的3D-TOF—MRA表现及其临床意义，以提高对该血管变异的认识。方法回顾分析45例有孔型基底动脉变异病例，所有病例均用3．0T磁共振行头部血管成像（MRA）检查。扫描源图像在GEADW4．3工作站上进行三维最大信号投影及容积再现重建，多角度旋转观察，全面分析异常血管情况。结果45例有孔型基底动脉变异均为单发，18例伴有其他颅内血管发育变异。有孔型基底动脉变异本组病例发生率约为0．83％，无性别、年龄差异，多见位于基底动脉近端、椎动脉汇合处，“开窗口”形态以小圆形、卵圆形及裂隙样为主，两侧开窗血管径可以等宽或不等宽，但均小于基底动脉直径。结论．3D-TOF—MRA可以准确显示有孔型基底动脉变异的位置、形态、毗邻关系及有无伴发其他血管性病变。明确诊断有孔型基底动脉变异，具有重要临床意义。Objective To describe the three-dimensional time-of-flight magnetic resonance angiography（3D-TOF-MRA） features of basilar artery fenestrations（BAF） and evaluate the clinical significance of its diagnosis. Methods A total of 5214 cases in which 3D-TOF-MRA was performed using a 3.0 Tesla scanner from November 2008 to May 2011 were retrospectively reviewed, and BAF was detected in 45 cases. Source images were processed using a workstation GE ADW4.3. Three diamensions maximum intensity projection （3D-MIP） and volume rendering（VR） images were obtained to displayed the abnormal blood vessel stereoscopically and viewed at different angles. Results All 45 BAFs were single, with 18 cases associated with other blood vessel developmental variation. The incidence of BAF was 0.83% in this group, without gender or age difference. The majority of BAFs were located at the proximal basilar artery or at the conjunction of bibateral arteria vertebralis, most with a small round, orbicular-ovate, or slit-like shape. Fenestration＇s limb calibers of both sides were equal or not, but were smaller than basal artery diameter. Conclusion 3D- TOF-MRA can display BAF clearly and exactly, including the location, shape, relationship with surrounding structures and other cerebral blood vessel variation. A positive diagnosis of BAF has significant clinical value.
