摘  要:目的探讨卵巢静脉在CT诊断盆腔肿瘤中的作用。方法以37例女性盆腔肿瘤患者为观察组,30例正常女性为对照组,2组均行强化CT检查。3位医师共同观察卵巢静脉显示情况、卵巢静脉最大径及通过卵巢静脉诊断盆腔肿瘤的准确性。结果研究组左侧卵巢静脉良好显示的占91.9%,右侧占89.2%,与对照组无统计学差异(P=1.01);卵巢肿瘤患侧卵巢静脉最大径(5.26±1.42)mm,较健侧增粗(P=0.038);通过卵巢静脉诊断盆腔肿瘤来源敏感度为81.3%,特异度85.7%,准确性83.8%,ROC曲线下的面积是0.88。结论通过追踪卵巢静脉可以准确判断盆腔肿瘤是否来源于卵巢。Objective To evaluate the value of the gonadal veins on multidetector CT( MDCT)in differentiating the origins of pelvic masses. Methods Three radiologists evaluated the CT contrast enhanced images of 37 female patients with pelvic mass and 30 healthy female controls. The maximum diameters of the gonadal veins were measured. The visualization of gonadal veins and origin determination based on anatomic continuity were also recorded. The sensitivity, specificity, and accuracy of ovarian origin determination were calculated. Results The detectability of the left and right gonadal veins in ovarian tumors were 91. 9% and 89. 2%. There was no significant difference between the mass and control groups (P=1.01). The diameter of the gonadal veins in the patients was (5.26±1.42) mm, which was larger than that in the control group (P=0. 038). The value of area under the ROC curve (Az) for ovarian origin determination was 0.88. The sensitivity, specificity, and accuracy were 81.3%, 85.7%, 83.8%, respectively. Conclusion Gonadal veins can be shown on MDCT with high consistency; MDCT provides an useful information for determining the origin of relevant pelvic tumors arising in females.
