糖尿病患者血浆中3种凝血标志物检测的临床意义

摘　　要：目的探讨糖尿病患者抗凝血酶Ⅲ(AT-Ⅲ)、D-二聚体(D-D)、血管性血友病因子(vWF)检测的意义。方法将82例糖尿病患者分为2组,糖尿病无血管并发症组40例,糖尿病微血管并发症组42例,入院时采集静脉血,进行vWF、D-D、AT-Ⅲ的检测;并设40例正常对照。结果糖尿病无血管并发症组患者血浆中vWF、D-D较正常对照组增高(P<0.05),糖尿病微血管并发症组患者血浆中vWF、D-D较糖尿病无血管并发症组及正常对照组升高,差异均有统计学意义(P<0.05)。②糖尿病无血管并发症组患者血浆中AT-Ⅲ活性与正常对照组比较,差异无统计学意义(P>0.05);糖尿病微血管并发症组患者血浆中AT-Ⅲ活性较糖尿病无血管并发症组及正常对照组升高,差异有统计学意义(P<0.05)。结论 AT-Ⅲ、D-D、vWF的异常与糖尿病微血管并发症的发生有密切关系,监测这几项指标对预测糖尿病微血管病变的发生有重要意义。[Objective] To explore the clinical significance of AT-Ⅲ、D-D and vWF detection in diabetic patients.[Methods]A total of 82 diabetic patients were divided into 2 groups.One group included the diabetic patients without angiopathy（40cases）.And another group contained the diabetic patients with angiopathy（42 cases）.Venous blood was collected before hospitalization for AT-Ⅲ,D-D and vWF determination.Forty healthy people were enrolled in the study as control group.[Results]① The levels of vWF and D-D in diabetic patients without angiopathy were higher than those of healthy people（P0.05）.Compared with both control group and patients without angiopathy,the levels of vWF and D-D were increased in the patients with angiopathy（P0.05）.②For the activity of AT-Ⅲ,there was no difference between the patients without angiopathy and the control group（P0.05）.However,the level of AT-Ⅲ in patients with angiopathy were different from the group without angiopathy or the control group（P0.05）.[Conclusion]The changes of AT-Ⅲ、D-D and vWF are closely related with microvascular complication of diabetes.Therefore,it is important to monitor these markers to predict the occurrence of diabetic vascular complications.
