妇科腹腔镜手术并发症发生率及其原因分析

摘　　要：目的:探讨妇科腹腔镜手术并发症的发生率、原因以及处理和预防措施。方法:回顾分析2006年5月—2010年12月同济大学附属第一妇婴保健院6347例行腹腔镜手术患者的临床资料。结果:6347例妇科腹腔镜手术中,91例出现并发症,发生率为1.43%(91/6347);91例中,经开腹处理12例。并发症包括出血18例,切口愈合不良17例,皮肤淤血15例,切口血肿和引流口出血13例,膀胱损伤9例,会阴部气肿9例,肠管损伤4例,输尿管损伤2例,颈管残端囊肿2例,臂丛神经损伤1例,下肢静脉血栓1例。结论:妇科腹腔镜手术并发症与手术难度和术者技巧有关,积极预防可降低其发生率,术中及时发现及正确处理可以改善预后。Objective：To investigate the incidence, causes, management and prevention of complications in laparoscopic gynecologic surgery. Methods：It was a retrospective analysis based on clinical data of 6347 cases of laparoscopic gynecologic surgery from May 2006 to December 2010. Results： Among the 6347 cases,complications occurred in 91 cases; the overall incidence was 1.43 % （91/6347）. Among the 91 cases, there were 18 cases of massive hemorrhage, 15 cases of skin congestion, 17 cases of abnormal incision healing, 13 cases of incision hematoma and drainage bleeding,9 cases of bladder injury, 9 cases of perineal emphysema,4 cases of intestinal injury,2 cases of ureter injury,2 cases of residual cervical cyst, 1 case of brachial plexus nerve injury and 1 case of lower limb thrombosis. Conclusions： Complications of laparoscopic gynecologic surgery are correlated with the difficulty of perforation and the proficiency of operator.
