新生儿院内获得性轮状病毒性肠炎25例临床分析

摘　　要：目的分析新生儿院内获得性轮状病毒性肠炎的临床特点及相关危险因素,并探讨其防治措施。方法统计该院新生儿科2010年1月至2011年12月院内感染的发生率,并对25例院内获得性轮状病毒性肠炎患儿的临床资料进行回顾性分析。结果 2010年1月至2011年12月该院新生儿科院内感染总发生率为2.6%,消化道感染占总感染的18.8%,消化道感染以轮状病毒性肠炎为主。25例院内获得性轮状病毒性肠炎患儿均在住院1周左右排黄色稀糊状粪便,每天4~6次不等,同时伴有不同程度发热及轻到中度脱水。所有患儿均无呕吐及惊厥发生,且通过给予静脉补液或口服加静脉补液等对症处理后,均在发病后3~5 d大便转正常。结论新生儿感染轮状病毒性肠炎以秋冬季发病为主,是新生儿院内消化道感染的主要类型,应积极采取有效措施,降低其发生率。Objective To investigate the clinical features,related risk factors,prevention and treatment of neonatal hospital-acquired rotavirus enteritis.Methods The incidence rate of neonatal nosocomial infection in the neonatal department of this hospital from January 2010 to December 2011 was calculated by statistics.The clinical data in 25 cases of hospital-acquired rotavirus enteritis were conducted the retrospective analysis on the risk factors,treatment and prognosis.Results The total incidence rate of nosocomial infection among hospitalized neonates during this period was 2.6%.In the infection positions,respiratory tract infection was dominated,accounting for 53.3%,followed by digestive tract infection（18.8%）,mainly rotavirus enteritis.25 cases all had yellow thin paste stool,4-6 times per day,in one week after hospitalization,accompanied by varying degrees of fever and mild to moderate dehydration.No vomiting and convulsions occurred.All 25 cases were back to normal stool within 3-5 d after symptomatic treatment of intravenous fluid infusion or oral plus intravenous rehydration.Conclusion Neonatal rotavirus enteritis occurs mostly in autumn and winter,and is the main type of neonatal nosocomial infection.The active and effective measures should be taken to reduce its incidence rate.
