高龄老年高血压患者尿IgG／肌酐独立相关因素的研究

摘　　要：目的探讨高龄老年（≥80岁）高血压患者IgG／肌酐（IgG／Cr）的独立相关因素，寻找减少高龄老年高血压患者IgG排出的方法。方法对1353例高龄老年高血压患者进行动态血压检测、血液生化指标检测、尿IgG和Cr检测，运用Logistic回归分析筛选高龄老年高血压患者IgG／Cr的独立相关因素。结果高龄老年高血压患者IgG／Cr的独立相关因素包括24h脉压（β=0．041，P〈001）、空腹血糖（β=0.210，P〈0．01）和超敏C反应蛋白（β=0．026，P〈0．05）。结论降低24h脉压、空腹血糖和超敏C反应蛋白水平，可能是减少高龄老年高血压患者IgG排出的重要方法。Objective To analyze the independent associated factors of urine immunoglobulin G/creatinine ratio （IgG/Cr） in hypertensive patients aged 80 and over. Methods A cross-sectional survey was conducted comprising a sample of 1 353 hypertensive patients aged 80 and over. All patients underwent 24h ambulatory blood pressure monitoring,laboratory measurement including urine IgG and Cr. Univariate analyses and Logistic analyses were applied to examine independent associated factors of IgG/Cr. Results The independent associated factors of IgG/Cr included 24h pulse pressure （β=0.041, P〈0.01）, glucose（β =0.210, P〈0.01）,high sensitivity C-reactive protein（β =0.026, P〈0.05）. Conclusion 24h pulse pressure,high blood glucose and sensitivity C-reactive protein levels are the risk factors for urinary IgG in hypertensive patients aged 80 and over.
