耻骨后保留尿道前列腺切除术应用体会

摘　　要：目的观察耻骨后保留尿道前列腺切除术（Madigan术式）治疗良性前列腺增生（benign prostatic hyperplasia，BPH）的疗效。方法69例患者接受Madigan术式行前列腺摘除术，对手术效果、并发症等进行1～2年术后随访。结果Madigan术式系直视下摘除增生腺叶，操作准确彻底、止血有效可靠，对前列腺段尿道可做到完整保留。随访期内手术效果确切满意，无尿失禁、尿道狭窄、性功能障碍等主要并发症发生。结论耻骨后保留尿道前列腺切除术治疗彻底，效果理想，术后并发症少而轻，无需特殊仪器设备，宜于在基层医院推广应用。Objective To investigate the clinical effects and complication of the improved prosta- tectomy with remaining the urethra by the back of pubis in treating the benign prostatic hyperplasia. Meth- od The clinical data of 69 cases received the improved prostatectomy with remaining the urethra by the back of pubis, and the clinical effects and complication of operation were observed. Results During this procedure the area of operation is exposed well and operation is exact simple and easy. After operation the patients who were recovers rapidly and less bleeding and complication was found. The patients were fol- lowed up for 1-2 years and satisfactory results has been achieved in al 1. Conclusion The treatment of improved prostatectomy with remaining the urethra by the back of pubis is thorough, the effect is ideal, complication after operation is fewer and the applicability is wide, so it is worth using in base hospital.
