通窄饮治疗冠脉介入术后阳虚血瘀证的临床研究

摘　　要：目的:观察通窄饮对冠脉介入术后阳虚血瘀证病人临床症状及相关细胞因子的影响。方法:将冠脉介入术后病人70例随机分为通窄饮组(治疗组)和常规西药组(对照组)各35例,二组均给西药常规扩冠、降脂、抗血小板聚集等治疗;中药治疗组加用通窄饮,每日1剂,8周为1个疗程。观察患者中医证候疗效、心绞痛疗效,治疗前后放免法检测肿瘤坏死因子(TNF-α)、白介素-6(IL-6)等细胞因子水平。结果:治疗组及对照组中医证候总有效率分别为82.9%与68.6%,治疗组明显优于对照组(P<0.05);治疗组与对照组心绞痛总有效率分别为85.7%、68.6%,两组间差异显著(P<0.05);治疗组治疗后TNF-α、IL-6均显著降低(P<0.05或P<0.01),两组间比较差异显著。结论:对冠脉介入术后阳虚血瘀证病人应用通窄饮可明显改善临床症状,降低TNF-α、IL-6等细胞因子分泌可能是其部分作用机制。Objective:To observe the effects of tongzhaiyin on clinical symptoms and related cytokines of patients with yang-deficiency and blood stasis syndrome after percutaneous coronary intervention.Methods:70 Cases after percutaneous coronary intervention was divided into tongzhaiyin treatment group and control group,two groups were given conventional western medicine treatment,treatment group was given tongzhaiyin to oral on tne basis of conventional western medicine treatment,one dose a day,eight weeks were one treatment course.clinical symptom were observed before and after treated,curative effect on angina pectoris was assessed,serum IL-6 and TNF-α levels were determined.Results:The effective rate of syndrome in treatment group was 82.9%,that was 68.5% in the control group,the effect of treatment group was obviously better than that of control group,the total effective rate of angina pectoris in treatemnt group was 82.9%,that was 71.4% in the control group,the difference between the two groups was significant.The levels of serum IL-6 and TNF-α were lowered significsntly after treated in treatment group,the changes of the two kinds of cytokines in control group were no obvious.Conclusion:Tongzhaiyin can significantly improve the clinical symptoms of patients after percutaneous coronary intervention,reducing IL-6 and TNF-α levels maybe part of the mechanism.
