社区人群中乙型肝炎病毒感染和代谢综合征的相关研究

摘　　要：目的:研究社区人群中乙型肝炎病毒(HBV)感染和代谢综合征(MS)的关系。方法:对参加2007-2009年间上海市普陀区各级医院的健康体检的常住人口进行调查,除了收集年龄、性别、腰围等数据外,还采集血清标本,检测其HBV携带情况以及空腹血糖、甘油三酯、高密度脂蛋白等MS相关指标,使用lo-gistic回归分析HBV感染和MS间的关系。结果:本次调查总数是9 414,乙型肝炎表面抗原阳性率是17.1%。乙型肝炎表面抗原在MS患病组中的阳性率为21.9%,明显比非MS患病组中的16.4%要高(P<0.000 1)。HBV感染和MS的联系粗的OR是1.561,95%可信区间是1.276～1.908,调整年龄和性别后,其调整OR为1.397,95%可信区间是1.139～1.714。结论:HBV感染和代谢综合征间存在一定的联系,研究结果表明HBV感染可能会增加研究人群代谢综合征的患病风险。Objective： To explore the association between metabolic syndrome（MS） and Hepatitis B Virus（HBV） infection based on community population.Methods： A community-based cross-sectional study design was adopted with people attended annual health check in multi-level hospitals from January 1 of 2007 to December 31 of 2009 being enrolled in Putuo District,Shanghai.Basic information such as age,gender,waist circumference were observed,blood samples were collected to detect the status of HBV infection and anthropometric measures related to MS,such as fasting blood sugar,triglyceride and high-density lipoprotein（HDL）.Logistic regression was used to estimate odds ratios and related 95% confidence intervals for the associations between MS and HBV infection.Results： A total of 9 414 subjects were included,the prevalence of HBsAg infection was 17.1%.Frequency of positive HBsAg infection in MS group was 21.9%,and was higher than the frequency（16.4%） in non-MS group（P0.000 1）.The crude OR between MS and HBV infection was 1.561,95% confidence interval（CI） was 1.276 to 1.908.Adjusted OR was 1.397,95%CI was 1.139 to 1.714 after age and gender were adjusted.Conclusion： There was association between MS and HBV infection.Results indicated that HBV infection might increase the prevalence of MS on community-based population.
