几种不同方法在人工流产术镇痛中应用的探讨

摘　　要：目的 :探讨一种安全、有效、简便可供选择的无痛人流方法。方法 :ASAⅠ级 2 5 0例中止妊娠者随机分为异丙酚组、氯胺酮组、宫术安栓组、宫颈旁阻滞组、对照组。观察总结了手术中镇痛效果、术后恢复等情况以及静脉麻醉组对呼吸循环的影响等。结果 :静脉麻醉组镇痛完全 (P <0 0 1) ,无人流综合症发生。局部用药组均不能完全消除疼痛及人流综合症的发生。异丙酚组对呼吸循环有一过性一定抑制作用 (P <0 0 5 ) ,氯胺酮组苏醒时间长 (P <0 0 1) ,苏醒时大多有不良反应发生 (P <0 0 1)。结论 :异丙酚作用迅速 ,效果完全 ,苏醒快 ,无不良反应。在麻醉医师操作监护下不失为一种安全、有效、方便、可行良好的无痛人流方法。Objective:To investigate a safe,effective,first of choice analgesia method used for artificial abortion Method:Two hundred and fifty ASAⅠ～Ⅱpatients undergoing elective artificial abortion.were randomly divided into propofol,ketamine,gongsuanshuan,paracervic block,and control groups.We observed analgesia effect and awakening conditions as well as the effects of the two intravenous anaesthesia on circulation and respiration.Result:Pain relief satisfactory in the intravenous groups( P <0 01)with no resultant artificial abortion syndrome.Local analgesia failed completely to eliminate pain and artificial abortion syndrome.There were transient circulatory and respiratory depression in propofol group( P <0 05).Awakening time was longer and the incidence of nausea and vomiting was apparently frequent in the ketamine group than in ropofol group( P <0 01).Conclusion:Propofol is still one of the most useful anestetic drugs in artificial abortion with complete efficacy,quick awakening and few side effects.
