颈臂丛联合阻滞在锁骨手术的临床观察

摘　　要：目的评价颈臂丛联合阻滞麻醉用于锁骨骨折内固定术的临床应用价值。方法锁骨手术120例,ASAI～Ⅱ级,随机分3组,分别采用臂丛颈丛联合阻滞、肌间沟臂丛阻滞、颈丛阻滞。观察麻醉效果以及有无中毒等不良反应。结果颈臂丛联合组麻醉阻滞效果最好,其优良率达100%,高于颈丛阻滞组(75.4%)和肌间沟臂丛阻滞组(64.2%)(P<0.05)。而3组不良反应对比则无明显差别(P>0.05)。结论颈臂丛联合阻滞用于锁骨骨折切开复位内固定术临床效果明显优于肌间沟臂丛阻滞、颈丛阻滞。Objective Assessment of cervical brachial plexus block anesthesia for internal fixation of clavicular fractures clinical application value.Methods 120 cases of clavicular operation,ASA Ⅰ ～ Ⅱ,were randomly divided into 3 groups,respectively,using cervical plexus block,brachial plexus interscalene brachial plexus block,cervical plexus block.Observation of anaesthetic effect and no toxic and adverse reaction.Results The brachial plexus block anesthesia combined group effect was the best.The excellent rate of 100%,higher than that of cervical plexus block group（75.4%） and interscalene brachial plexus block group（64.2%）（P 0.05）.And the 3 group of adverse reactions of no significant difference（P 0.05）.Conclusion Cervical brachial plexus block for clavicular fractures with open reduction and internal fixation clinical effect was better than that of interscalene brachial plexus block,cervical plexus block.
