促排卵周期小剂量黄体酮联合绒毛膜促性腺激素触发排卵的临床观察

摘　　要：目的观察促排卵周期小剂量黄体酮联合绒毛膜促性腺激素(HCG)触发排卵的临床效果,探讨提高周期排卵率及妊娠率的方法。方法选择不孕症排卵障碍患者160例,随机分为2组:观察组84例,176个周期;对照组76例,158个周期。于促排卵周期观察组给予黄体酮5 mg+HCG 10 000 IU肌肉注射,对照组给予HCG 10 000 IU肌肉注射,随后每日B超检查是否排卵。排卵后14 d检测血或尿HCG,以确定是否妊娠。结果观察组触发排卵176个周期,165个周期成功排卵,排卵率93.4%;44个周期妊娠,周期妊娠率为26.7%,总妊娠率52.4%。对照组触发排卵158个周期,116个周期成功排卵,排卵率73.4%;20个周期妊娠,周期妊娠率为17.2%,总妊娠率26.3%。2组总妊娠率及排卵率相比均有显著性差异(P均<0.05)。结论小剂量黄体酮联合绒促性素触发排卵方案可有效提高排卵率及总妊娠率。Objective It is to observe the clinical effect of small doses of progesterone combined with HCG to trigger ovulation in ovulation cycle and to explore the methods for improving cycle-ovulation rate and pregnancy rate.Methods 160 cases of infertility with ovulation disorders were randomly divided into two groups： observation group of 84 cases,176 cycles;control group of 76 cases,158 cycles.In ovulation cycle,the observation group was treated with progesterone 5mg＋HCG 10 000 IU by intramuscular injection,and the control group was given HCG 10 000 IU by intramuscular injection.Followed by,to examine whether ovulation by B ultrasound one time per day.At 14 days after ovulation,the serum or urine HCG were examined to determine whether pregnancy.Results In the observation group,there were 176 trigger ovulation cycles and 165 cycles of successful ovulation,and the ovulation rate was 93.4%;there were 44 cycle pregnancis,and cycle pregnancy rate was 26.7% and the total pregnancy rate was 52.4%.In the control group,there were 158 trigger ovulation cycles and 116 cycles of successful ovulation,and the ovulation rate was 73.4%;there were 20 cycle pregnancis,cycle pregnancy rate was 17.2%,and the total pregnancy rate was 26.3%.There was significantly statistical difference between two groups in the ovulation rate and the pregnancy rate（P0.05）.Conclusion Small doses of progesterone combined with HCG trigger ovulation program can effectively increase the ovulation rate and the pregnancy rate.
