难治性癫痫中医证候的判别分析

摘　　要：目的:研究难治性癫痫的中医分型,探讨难治性癫痫中医证候诊断规律。方法:收集76例难治性癫痫患者,根据相关中医诊断标准,将患者分别诊断为肝郁痰凝、肾虚精亏、心脾两虚、气滞血瘀等4型。将临床症状分为轻度、中度、重度和严重4级,分别记为1-4分。根据患者中医证型对变量进行逐步判别分析。结果:根据难治性癫痫中医证型,使用逐步判别分析方法,筛选出对区分4类中医证型有显著贡献的28个变量,通过这些变量建立难治性癫痫中医证型判别函数;该函数具有非常显著的判别效果(P<0.01)。结论:根据患者临床表现建立起的难治性癫痫中医证型判别函数与临床实际吻合良好,判别分析方法能够提高难治性癫痫中医诊断和鉴别诊断的客观性和准确性,有较好的应用前景。objective： To analyse the intractable epilepsy patients＇ TCM syndrome by the way of discriminative analysis,and explore diagnosis rule of TCM syndrome.Methods： We researched 76 intractable epilepsy patients,and diagnosed them as liver qi depression and phlegm stasis syndrome,kidney and marrow deficiency syndrome,heart and spleen deficiency syndrome,qi and blood stasis syndrome.We divided their symptoms as light,media,heavy and very heavy,and recorded as 1,2,3 and 4 score.Then we made a stepwise discriminative analysis to their symptom variables based on their TCM syndrome.Results： Based on intractable epilepsy patients＇ TCM syndrome,we obtained 28 variables from patients＇ symptoms by the way of stepwise discriminative analysis（P0.01）.Then we found intractable epilepsy patients＇ TCM syndrome discriminative function through all these 28 variables.And these discriminative functions had significantly discriminative affection（P0.01）.Conclusion： The function was accorded with intractable epilepsy clinical facts.And the way of ration analysis,as discriminative affection,could improve the objectivity and accuracy of intractable epilepsy patients＇ TCM diagnosis and differentiation diagnosis,which has a good application vista.
