社区规范管理对老年原发性高血压合并2型糖尿病患者血压及血糖的影响

摘　　要：目的探讨社区规范管理对老年原发性高血压合并2型糖尿病患者血压及血糖的影响。方法选择本社区的104例老年原发性高血压合并2型糖尿病患者为研究对象,运用随机数字表法分为对照组和观察组,各52例,对照组仅进行普通治疗和一般宣教,观察组在上述干预的基础上进行健康教育、膳食运动指导等综合干预措施。进行12个月的随访,比较两组患者血糖达标率、血压达标率及再住院率。结果经过12个月的社区健康服务中心管理干预,观察组血压及血糖达标率明显高于对照组,差异有统计学意义(P<0.05);观察组患者饮食控制率、适当运动率、自我监测率及遵医行为率均明显高于对照组,差异有统计学意义(P<0.05)。结论对老年原发性高血压合并2型糖尿病患者实施社区医院管理干预可有效地控制患者的血压及血糖,提高老年原发性高血压合并2型糖尿病患者的血压、血糖达标率,值得在社区推广应用。Objective To discuss the influence of community health service center management on the blood pressure and blood glucose of elderly hypertensive patients with type 2 diabetes mellitus.Methods 104 elderly patients of primary hypertension complicated with type 2 diabetes mellitus were selected in the communityl,and divided into control group and observation group using a random number table,52 cases each.The control group was only given ordinary therapy and general education,while the observatin group was given health education,dietery and exercise guidance based on the above interventions.12 months of follow-up was done,and of the standard compliance rate of blood pressure and blood glucose,and rehospitalization of the two groups were compared.Results After 12 months of community health service center management intervention,the blood pressure and blood glucose compliance rate of the observation group was higher than that of the control group,the difference had statistical significance（P0.05）.The diet control rate,the appropriate rate of motion,self monitoring rate and compliance rate of the observation group were higher than the control group,the differences had statistical significance（P0.05）.Conclusion Community hospital management interventions to elderly primary hypertension patients complicated with type 2 diabetes mellituscan effectively control blood pressure and monitor blood glucose,increasing the compliance rate,thus is worthy of popularization and application in the community.
