髓鞘碱性蛋白水平在不同OCSP亚型老年急性脑梗死患者血清中的变化

摘　　要：目的:观察不同临床亚型老年急性脑梗死(ACI)患者血清髓鞘碱性蛋白(MBP)水平变化及其与预后的关系。方法:采用ELISA法检测328例连续收住入院、发病在3d内的老年ACI患者血清MBP水平,并按照牛津郡社区卒中计划(OCSP)分型标准分为4大亚型并行NIHSS、mRS评分,分析不同OCSP亚型MBP水平变化及其与预后相关性。结果:OCSP分型中完全前循环型脑梗死(TACI)、部分前循环型脑梗死(PACI)、后循环型脑梗死(POCI)MBP水平升高,腔隙性脑梗死(LACI)变化不明显;与对照组比较,TACI、PACI及POCI亚型有显著性差异(P<0.05);LACI亚型90天时mRS 0-2分患者比率最高,其次为PACI、POCI亚型,而TACI亚型90天时mRS 0-2分患者比率最低。MBP与TC、TG、LD-C、SBP、DBP、年龄呈正相关,与HDL呈负相关。结论:MBP随OCSP亚型的不同而变化,发病后血清MBP可以作为老年急性脑梗死患者预后判断的指标之一。Objective： To investigate pattern of fluctuations and clinical significance of myelin basic protein（MBP） levels in patient with different subtypes of acute cerebral infarction and its relationship with the score of mRS.Methods：MBP level was measured in 328 ACI patients by ELISA.All ACI patients were classified into 4 groups based on the Oxfordshire community stroke project（OCSP） criteria.And then the relationships between MBP level and mRS in different OCSP subtypes were analyzed.Results：The serum level of MBP in TACI,PACI and POCI increased after cerebral infarction,the serum level of MBP of LACI subtype had no significant changes in OCSP.Compared to the control group,the serum level of MBP in TACI,PACI and POCI were significant different from each other（P0.05）.mRSO-2 score of LACI subtype was the highest,followed by that in PACI、POCI subtype,and it was the lowest in TACI subtype.MBP was positively correlated with TC,TG,LD-C,SBP,DBP,age,while negatively correlated with HDL.Conclusions：MBP level changes with different classifications of subtype and it can be marker indicating severity of cerebral infarction in elderly patients.
