三餐前短效胰岛素加甘精胰岛素和胰岛素泵短期强化治疗对2型糖尿病疗效对比

摘　　要：目的:对比3餐前短效胰岛素加甘精胰岛素和胰岛素泵短期强化治疗对2型糖尿病的治疗情况。方法:选取2011年5月～2012年5月在我院内分泌科门诊诊断的2型糖尿病患者50例,随机分为两组,A组为3餐前短效胰岛素加甘精胰岛素组,B组为胰岛素泵组。连续给药12周后,比较治疗前、后两组患者空腹、餐后2h血糖水平,胰岛素用量,血糖达标时间,治疗成本及低血糖发生次数。结果:两种治疗方式均能降低2型糖尿病患者的血糖水平,血糖达标时间、空腹、餐后2h血糖水平及糖化血红蛋白差异均无统计学意义(P>0.05)。A组低血糖发生次数较B组明显增多(P<0.05),胰岛素用量及治疗成本明显低于B组(P<0.05)。结论:3餐前短效胰岛素加甘精胰岛素和胰岛素泵均能有效降低2型糖尿病患者的血糖水平,前者大大降低了胰岛素用量及治疗成本,但低血糖发生率较高。Objective： To compare the efficacy of glargine insulin with pre-meal short-effective insulin and continuous subcutaneous insulin infusion（CSII） on type 2 diabetes.Methods：A total of 50 patients with type 2 diabetes that admitted to the Department of Endocrinology of our hospital during May 2011 and May 2012 were randomly divided into group A and group B with 50 cases in each group.Group A was treated with intensive insulin delivery of glargine insulin plus RI each meal and CSII,while group B was given insulin pump.After a consecutive treatment of 12 weeks,level of FBG、2h PBG and insulin consuming volume and time for blood glucose restoring to be normal were compared between the two groups.Results： Both methods can decrease blood glucose level,and no significant difference in time for blood glucose restoring to be normal,level of FBG,2h PBG or glycolated hemoglobin was found between the two groups（P0.05）.But hypoglycemia was observed significantly more frequently in group A than that in group B（P0.05）,insulin consuming volume and cost of group A was less than that of group B（P0.05）.Conclusion：Both basal glargine with pre-meal short-effective insulin and continuous subcutaneous insulin infusion can effectively control hyperglycemia level and the former method can reduce insulin consuming dosage and hospitalization costs though incidence of hypoglycemia was high.
