探讨长骨骨干骨折内固定术后断裂及松动的原因

摘　　要：目的探讨四肢长骨骨干骨折内固定术后内固定物断裂、松动原因与对策。方法通过回顾性分析该院自2008年5月至2012年1月切开复位钢板固定治疗的四肢骨干骨折136例:肱骨干骨折24例;尺桡骨骨干骨折13例;股骨干骨折43例;胫腓骨骨干骨折56例。评估骨折的伤情及术中、术后处理与内固定断裂的相关性。结果随访6个月至3年,平均1年,6例出现钢板断裂;其中4例合并螺钉松动。结论合理的选择、使用内固定器械,恰当的复位与植骨及术后正确的功能锻炼、定期随访,能有效防止内固定物断裂或松动。Objective To study the cause of the refracture of the bone fixated with plate postoperation.Methods From May 2008 to January 2012,136 cases of limb fracture who were treated with internal fixators in this hospital were followed up and analyzed：24 cases in humeral shaft fracture,13 in ulnar and radial shaft fractures,43 in femoral shaft fracture,56 in tibial shaft fracture.Results The cases were followed up from six months to three years（mean of one year）,plate breakage in 6 cases,and among 4 patients with screw loosening.Conclusion According to the conditions of patients,the appropriate use of the Internal fixation,necessary bone graft and correct rehabilitation during postoperation are valuable procedures for the prevention of implant breakage and loosening.
