摘　　要：依托基于区域医疗的慢性心血管疾病一体化诊疗平台,开展心血管疾病新型医疗服务模式在基层卫生机构的应用,在促进医疗卫生资源的合理分布和利用、方便基层重症患者就医看病、提高基层卫生机构的诊断水平和覆盖率、降低医疗费用、和谐医患关系等方面均发挥了积极的作用,为患者、主诊医生、基层卫生机构等带来了良好的效益。Relying on the Integration of diagnosis and treatment platform in regional medical services for the chronic cardiovascular disease, it is to carry out the use of the new model of health care services in primary health institutions for the cardiovascular disease. This would promote the rational distribution of the health resources, be accessible for the primary severe patients to receive the medical treatment and play an active role in the improvement of the diagnostic skill and coverage of primary health, the reduction of the medical costs as well as the harmony relationship between doctors and patients, etc. It also brings the good benefits for patients, the attending doctors, and the primary health institutions.
