耐甲氧西林金黄色葡萄球菌的SCCmec分型及耐药性研究

摘  要:目的研究滨州某医院临床分离的耐甲氧西林金黄色葡萄球菌SCCmec基因型分布特点及耐药状况。方法应用多重聚合酶链式反应(PCR)技术对临床分离的65株MRSA进行SCCmec基因分型;普通PCR对MRSA进行4种耐药基因扩增;应用K-B纸片扩散法对19种抗生素进行药敏试验。结果 SCCmec分型显示58株为SCCmecⅢ型的MRSA;5株为Ⅱ型;另外2株不能分型。65株MRSA临床株对多种抗生素均耐药。4种耐药基因的阳性率分别为aac(6')/aph(2″)93.85%,aph(3')Ⅲ为52.31%,tetM基因的阳性率为92.31%,ant(4')为9.23%。结论研究结果显示医院分离的MRSA对氨基糖苷类、四环素类耐药基因有较高的携带率,且具有多重耐药的特征,SCCmec基因型以Ⅲ型为主,多重PCR法可以有效的用于临床分离的MRSA的SCCmec分型研究。Objective To investigate genotype characteristics of staphylococcal cassette chromosome mec(SCCmec) and antimicrobial resistance of methicillin-resistant Staphylococcus aureus(MRSA) isolated from one hospital of Binzhou.Methods Multiplex PCR was used to detect mecA gene.Antibiotic-resistant genes of aac(6')/aph(2''),aph(3')Ⅲ,tetM,and ant(4') were analyzed by traditional PCR.Antimicrobial susceptibility of 19 antibiotics was detected by Kirby-Bauer method.Results The isolates were almost SCCmec Ⅲ positive,only 5 isolates were SCCmecⅡ,2 isolates were not genotyped.All of the testing strains were susceptible to linezolid,quinupristin,vancomycin,teicoplanin and lower resistant rate to chloramphenicol.The positive rates of aac(6')/aph(2''),aph(3')Ⅲ,tetM and ant(4') were 93.85%,52.31%,92.31and 9.23%.Conclusions There were high positive percentages of antibiotic resistant genes of aac(6')/aph(2'') and tetM in the isolates of MRSA.MRSA had multiple resistances to commonly used antibiotics.Almost all genotypes of MRSA from clinical source carry the SCCmecⅢ gene.Multiplex PCR can be applied into genotyping study of MRSA SCCmec effectively.
