硝普钠和硝酸甘油治疗急性左心衰竭临床疗效比较

摘　　要：目的观察硝普钠和硝酸甘油治疗急性左心衰竭的临床疗效。方法选择2009年1月至2011年12月云南省第二人民医院心内科住院治疗的急性左心衰竭患者120例，随机分为硝普钠组和硝酸甘油组各60例，观察两组的临床疗效。结果治疗后两组患者的收缩压、舒张压、心率及尿量均优于治疗前（P〈0．05）。治疗后硝普钠组患者在收缩压、舒张压、心率及尿量改善方面优于硝酸甘油组（P〈0．05）。硝普钠组总有效率为93．3％，硝酸甘油组为78．3％（P〈0．05）。硝普钠改善心衰症状所需的时间更短。两组患者均未见明显的药物不良反应。结论在抢救急性左心衰时硝普钠较硝酸甘油更有优势。Objective To observe the effect of sodium nitroprusside and nitroglycerin in treating patients with acute left ventricular failure. Methods 120 cases with acute left ventricular failure in the department of car-diology of Yunnan No.2 People＇s Hospital from January in 2009 to December in 2011 were divided into the sodi-um nitroprusside group and the nitroglycerin group. The clinical effects were observed. Results The systolic pres-sure, diastolic pressure, heart rate and urine volume in the two groups after treatment were superior to those be- fore treatment with significant difference respectively （P〈0.05）. The systolic pressure, diastolic pressure, heart rate and urine volume in the sodium nitroprusside group were superior to those in nitroglycerin group with significant difference respectively （P〈0.05）. The total effective rate was 93.3% in the sodium nitroprusside group and 78.3% in the nitroglycerin group with significant difference respectively （P〈0.05）. The sodium nitroprusside group work fasterly than the nitroglycerin group in improving symptoms of heart failure. There was no significant difference in adverse drug reactions between two groups. Conclusion The sodium nitroprusside is superior to nitroglycerin in treating patients with acute left ventricular failure, which is worth of clinical application.
