虫草制剂对慢性肾脏病3期和4期患者营养不良状况的影响

摘　　要：目的观察虫草制剂对慢性肾脏病3、4期患者营养不良状况的影响。方法采用SAS 9.1统计学软件产生伪随机数列(种子数为20120604),将48例慢性肾脏病3、4期患者〔27例男性,平均年龄为(52.7±10.4)岁〕随机分配至研究组(n=24)和对照组(n=24),研究组给予低蛋白饮食+百令胶囊治疗,对照组给予低蛋白饮食+开同片治疗,两组均给予西医基础治疗,包括利尿、控制血压、调节钙磷代谢、纠正酸中毒及电解质紊乱等治疗。两组疗程均为12周,治疗前后比较两组患者的主观全面评价量表(SGA)评分等级、人体测量指标〔包括身高、体质量、体质指数(BMI)、三头肌皮褶厚度(TSF)、上臂围(MAC)、上臂肌围(MAMC)〕、血生化指标〔尿素氮(BUN)、血肌酐(Scr)、估算的肾小球滤过率(eGFR)、血清清蛋白(Alb)、血清前清蛋白(PA)、转铁蛋白(Tf)、总胆固醇(TC)、胰岛素样生长因子-1(IGF-1)〕。结果治疗前两组患者的SGA评分等级、人体测量指标、血生化指标比较,差异均无统计学意义(P>0.05);治疗后研究组患者的血清PA、IGF-1水平均高于对照组,差异有统计学意义(P<0.05),其他指标间差异均无统计学意义(P>0.05)。结论虫草制剂能够升高慢性肾脏病3、4期患者的血清PA和IGF-1水平,改善患者的营养不良状态。Objective To observe the effects of preparation of Cordyceps on improving malnutrition in patients with chronic kidney disease in stages Ⅲ-Ⅳ.Methods 48 patients（27 males with average age of 52.7±10.42 years） with chronic kidney disease in stages Ⅲ-Ⅳ were randomly divided into observation group（24 cases） and control group（24 cases） by Statistical Analysis System（Version 9.1） software（seed=20120604）.The observation group was given low-protein diet and Bailing capsules,while the control group was given low-protein diet and compound α-Ketoacid tablets.Besides,both groups were given basic therapies,including diuretic therapy,blood pressure control,regulation of calcium and phosphorus metabolism,correction of acidosis and electrolyte imbalance.The course of treatment was 12 weeks for both groups.The subjective global assessment（SGA） grading,body measurement indicators（height,body weight,BMI,TSF,MAC and MAMC） and blood biochemical indicators（BUN,Scr,eGFR,Alb,PA,Tf,TC and IGF-1） were compared between the two groups before and after treatment.Results The SGA grading,body measurement indicators and blood biochemical indicators between the two groups before treatment showed no statistically significant differences（P0.05）.After treatment,the levels of PA and IGF-1 in observation group were significantly higher than those of control group（P0.05）,but other indicators showed no statistically significant differences（P0.05）.Conclusion Preparation of Cordyceps can raise serum prealbumin and IGF-1 levels in patients with chronic kidney disease of stages Ⅲ-Ⅳ and improve the malnutrition condition.
