社区综合干预对糖尿病前期患者病情进展的影响研究

摘　　要：目的探讨社区综合干预对糖尿病前期患者病情进展的影响。方法选择2009—2011年在北京市昌平区北七家镇18岁以上社区居民体检中发现的空腹血糖受损(IFG)和(或)糖耐量受损(IGT)患者163例,按患者个人意愿分为对照组(70例)和干预组(93例),对照组给予常规降糖治疗,干预组在此基础上进行社区综合干预(饮食、运动指导、纠正不良生活习惯、降压、降糖治疗)。比较治疗前后两组患者血糖、血压及血脂的变化。结果两组治疗前血糖、血压及血脂的各指标比较,差异均无统计学意义(P>0.05)。干预组治疗后空腹血糖、餐后2 h血糖及低密度脂蛋白胆固醇水平均较对照组降低,差异有统计学意义(P<0.05);而体质指数、收缩压、高密度脂蛋白胆固醇、总胆固醇及三酰甘油水平与治疗前比较,差异无统计学意义(P>0.05)。结论对糖尿病前期者有针对性地进行社区综合干预,可以较常规治疗更有效地防止或延缓糖尿病前期发展为糖尿病的进程。Objectives To explore the effect of community comprehensive intervention on pre-diabetes patients.Methods A total of 163 patients with impaired fasting glucose（IFG） and（or） impaired glucose tolerance（IGT）,aged over 18 years old,were divided,according to personal willingness,into groups control（n=70,given conventional hypoglycemic therapy）,intervention（n=93,given,based on conventional,community comprehensive intervention such as diet,exercise guidance,bad life habits correcting）.The changes of blood glucose（BG）,blood pressure（BP） and blood lipids were compared between 2 groups before and after treatment.Results No significant difference was noted in BG,BP,blood lipids between 2 groups before treatment（P0.05）.Levels of FBG,2 h postprandial BG and low density lipoprotein cholesterol were lower in group intervention than in control after treatment,the difference was significant（P0.05）,but there was not difference in body mass index,systolic blood pressure,high density lipoprotein cholesterol,cholesterol and triglyceride（P0.05）.Conclusion Targeted community comprehensive intervention based on conventional treatment is more effective on prevention or delay of prediabetes progressing to diabetes than conventional treatment.
