不同年龄段脑梗死患者颅外颈动脉粥样硬化特点分析

摘　　要：目的探讨不同年龄段脑梗死患者颅外颈动脉粥样硬化的特点及其与脑梗死之间的关系。方法将258例脑梗死患者根据世界卫生组织年龄划分标准分为3组:青年组(58例)、中年组(89例)和老年组(111例)。3组患者均于入院后行颈动脉彩色多普勒超声检查,分析不同年龄段脑梗死患者颅外颈动脉粥样硬化的特点。结果 3组患者颅外颈动脉粥样硬化随年龄增加而显著增加,老年组最高,中年组次之,青年组则最低,差异有统计学意义(P<0.01)。3组患者颅外颈动脉粥样硬化斑块发生率以老年组最高,中年组次之,青年组则最低,差异有统计学意义(P<0.01)。3组颅外颈动脉粥样硬化软斑块发生率以青年组最高,老年组次之,中年组则最低,差异有统计学意义(P<0.05)。结论不同年龄段患者颅外颈动脉都有粥样硬化,且随年龄增加而显著增加。不稳定性软斑块不仅是引起中老年人群脑梗死以及脑梗死反复发作的重要原因之一,而且也是青年脑梗死的主要原因之一,监测颅外颈动脉硬化及斑块对各年龄段人群都具有重要的意义。Objective To explore the relationship between characteristics of extracranial carotid atherosclerosis and cerebral infarction in patients of different ages.Methods According to the World Health Organization Classification,258 patients with cerebral infarction were divided into three groups：young group（n=58）,middle-aged group（n=89） and older group（n=111）.Color Doppler ultrasonography of carotid artery was performed after admission to analyze the characteristics of extracranial carotid atherosclerosis in patients of different ages.Results The prevalence of extracranial carotid atherosclerosis and the incidence of extracranial carotid artery atherosclerotic plaques significantly increased with age（P0.01）.However,the incidence of extracranial carotid atherosclerotic soft plaques obviously decreased in the order of young groupolder groupmiddle-aged group（P0.05）.Conclusion The prevalence of extracranial carotid atherosclerosis increases with age in patients with cerebral infarction.The instability of soft plaques is a reason for cerebral infarction and recurrent attack not only in elderly population but also in youth population.Therefore,the monitoring of extracranial carotid atherosclerosis and plaques is of great significance in patients of different ages.
