摘 要:目的探讨老年尿路感染患者尿液分离阿萨希毛孢子菌的基因分型及其药敏特征,为临床治疗提供依据。方法共收集2009年1月至2012年1月尿路感染阿萨希毛孢子菌10株,设计序列特异性引物对其基因进行鉴定,并采用微量肉汤稀释法和E-test法对其进行药敏试验。结果基因型别鉴定结果显示,10株阿萨希毛孢子菌以Ⅳ型基因为主,占70%,其次分别为Ⅲ型(20%)和Ⅰ型(10%);体外药敏试验结果显示,各菌株对两性霉素B、氟康唑、伏立康唑、伊曲康唑、5′-氟胞嘧啶均为敏感,其中伏立康唑的最低抑菌浓度最小,且部分菌株对伊曲康唑呈现剂量依赖性敏感。结论老年尿路感染患者尿液阿萨希毛孢子菌基因型别以Ⅳ型为主,三唑类药物对其具有显著疗效,其中以伏立康唑效果最佳。Objective To investigate common genotypes and drug sensitivity of Trichosporon asahii(T, asahii) isolated from elder urinary tract infection patients. Methods Ten T. asahii urinary tract infection(UTI) isolates were collected between January 2009 and January 2012. Sequence specific primers were designed to identify and genotype these clinical strains. Drug sensitivity tests were carried out according to the reference method for broth dilution antifungal susceptibility testing of yeasts,approved standard- Third edition,as well as E-test method. Results The ten isolates belonged to type I , III and IV , respectively. Among them, type IV was the most prevalent one, accounting for 70%, then was type III (20%) and type I (10%). Drug sensitivity test results suggested that all strains were sensitive to Amphotericin 13, Fluconazole,ltraconazole,Vorieonazole and 5-Flueytosine except that some ones were dosedependent sensitivity against Itraconazle. Conclusion Type IV is the most dominant genotype of T. asahiis isolated from elder UTI patients. Voriconazole is the most effective in treating UTI caused by T. asahiis.
