吉林省危重新生儿转运现状分析

摘　　要：目的:探讨吉林省新生儿转运系统对降低该省新生儿死亡率、提高存活率的积极作用及存在的问题。方法:回顾性分析吉林大学第一医院新生儿科转运的1228例新生儿,其中胎龄≤32周的早产儿167例,32周<胎龄<37周的早产儿588例,胎龄≥37的足月儿473例;分析疾病组成、治疗转归以及转运地区及转运科室组成的变化。结果:转运患儿逐年增多,2010年共转运547例,而2011年转运681例,无1例转运途中死亡。治愈及好转1 131例占92.1%,放弃治疗60例占4.89%,死亡37例占3.01%。2011年市内转运比例增加,同时市内其他医院产科转运所占比例较儿科转运所占比例增加。结论:吉林省新生儿转运逐年发展完善,降低了该省危重新生儿的病死率,改善了预后。但存在高危儿生后二次转运的问题。Objective： To explore the active effect of neonatal emergency transfer system （NETS） in Jilin province on reducing neonatal mortality and increasing neonatal survival rate and the existing problems. Methods： A total of 1 228 neonates transferred into department of neonatology in the hospital were analyzed retrospectively, including 167 premature infants with gestational age ≤32 weeks, 588 premature infants with gestational age within 32 -37 weeks, and 473 premature infants with gestational age ≥37 weeks. The changes of pro- portion of diseases, outcome, areas, and departments were analyzed. Results： The ill neonates transferred increased year by year, in 2010, 547 ill neonates were transferred, while in 2011,681 ill neonates were transferred, none died during transfer. A total of 1 131 neonates were cured or improved, accounting for 92. 1% ; 60 neonates were given up, accounting for 4. 89% ; 37 neonates died, accounting for 3.01%. In 201i 1, the proportion of transferred neonates in urban areas increased, and the proportion of transferred neonates from obstetrical department in urban areas was higher than that from pediatric department. Conclusion： NETS in Jilin province develops and improves year by year, which reduce the case fatality rate of critically ill neonates and improve the prognosis, but there is a problem of secondary transfer of high risk neonates after birth.
