社区心血管疾病护理路径的现状与进展

摘　　要：心血管疾病作为一种慢性疾病,社区护理成为该疾病护理干预的重要部分。如何使社区护理路径更规范、更系统、更科学的开展,一直是心血管护理领域的重要话题。目前的社区心血管疾病护理仍存在着许多问题,缺乏统一、有效的管理体制,社区专业医疗资源缺乏、专业知识、人才不足等,严重限制了社区护理的发展。建立三级管理体制,利用远程监测系统,完善社区患者电子档案,充分利用网络技术,使大型三甲医院的丰富医疗资源覆盖到社区或其他地区,这将成为今后发展的重要方向。Community nursing has become a very important part of cardiovascular disease＇ s major interven- tion. It has always been a pressing matter to discover a more standardized, systemic, and scientific breakthrough of car- diovaseular disease （CVD） nursing. Currently, community CVD nursing still contains many problems, including lack of unified and effective management, as well as lack of specific resources, knowledge, and human resources, which se- verely limit community nursing development. An important direction for nursing development would be spreading the resources of Triple A hospitals to communities or remote areas by compiling a tertiary management mechanism utiliz- ing remote monitoring system, completing patients＇ electronic case report, as well as taking full advantage of interact technology.
