阿托伐他汀钙治疗脑梗死并发颈动脉粥样硬化斑块49例

摘　　要：目的探讨阿托伐他汀钙对脑梗死并发颈动脉粥样硬化斑块疗效及对C反应蛋白(CRP)的影响。方法脑梗死并发颈动脉粥样硬化斑块的患者98例,随机分为治疗组和对照组各49例。两组常规治疗相同,对照组予以硫酸氯吡格雷片75 mg顿服;治疗组在对照组基础上加用阿托伐他汀钙20 mg,qd,po。两组患者疗程均为8周。结果治疗组在治疗后血脂水平明显改善(P<0.05或P<0.01),并显著优于对照组(P<0.05或P<0.01);两组治疗后斑块积分较治疗前均降低(P<0.05或P<0.01),治疗组下降更为明显(P<0.05);治疗组和对照组斑块有效率分别为91.84%,77.55%(P<0.05);治疗后两组CRP水平与治疗前均明显下降(P<0.01),且治疗组下降更为显著(P<0.01)。结论阿托伐他汀钙能明显改善脑梗死并发颈动脉粥样硬化斑块患者的血脂、血清CRP水平,能延缓动脉粥样硬化的进展,甚至可使动脉斑块缩小。Objective To investigate the effect of atotvastatin calcium on cerebral infarction with carotid atherosclerosis and its influence on C-reaction protein （CRP）. Methods Ninety-eight cases with cerebral infarction and carotid atherosclerosis were randomly divided into treatment group and control group （n = 49 of each）, All the patients were orally administered with clopidogrel hydrogen sulfate tablets （75 rag）. The patients in the treatment group were additionally treated with atorvastatin calcium （20 mg,qd. ,po）. Treatment course of both groups was 8 weeks. Results After treatment, blood lipid levels of the treatment group were significantly alleviated （ P〈0.05 or P〈0.01 ）, with significant difference as compared with those of the control group （P〈0.05 or P〈0.01 ） ;plaque score was significantly decreased in both groups （P〈0.05 or P〈0.01 ）, with significant difference between the two groups （P〈0, 05） ;effective rate of plaque therapy was 91.84% and 77.55% in the treatment group and control group, respectively （ P 〈 0. 05 ）. After treatment, CRP level was significantly decreased in both groups, and it was decreased more significantly in the treatment group （ P〈0.01 ）. Conclusion Atorvastatin calcium can significantly improve the levels of blood lipids and serum CRP, and the use of statins can delay the progress of atherosclerosis and even reduce plaque.
