摘 要:目的观察右美托咪定复合硬膜外麻醉用于妇科手术的临床效果。方法选取择期经下腹部子宫肌瘤手术患者74例,随机分为治疗组和对照组各37例。治疗组硬膜外腔给予0.75%左布比卡因15 mL与右美托咪定0.1μg.kg-1,对照组给予0.75%左布比卡因15 mL。比较两组感觉阻滞效果、运动阻滞效果。结果两组患者的感觉阻滞起效时间和最高感觉阻滞时间均差异无统计学意义(P>0.05);治疗组和对照组感觉阻滞维持时间分别为(194.2±66.8),(165.2±69.8)min(P<0.05);两组运动阻滞起效时间和最大运动阻滞分布均差异无统计学意义(P>0.05);治疗组和对照组运动阻滞恢复时间分别为(373±27),(315±26)min(P<0.05),最大运动阻滞时间分别为(22.7±4.6),(25.7±5.8)min(P<0.05)。结论右美托咪定联合左布比卡因可延长感觉阻滞和运动阻滞维持的时间,运动阻滞和镇痛作用均增强,麻醉效果更加完善。Objective To observe the effects of epidural anesthesia compound dexmedetomidine on the patients who underwent gynecologic surgery. Methods Seventy-four patients undergoing elective hysterectomy for uterine fibroids were randomly divided into two groups. Dexmedetomidine group (n = 37 ) was given 0. 75% levobupivacaine 15 mL and dexmedetomidine 0.1 μg· kg-1 for epidural anesthesia; control group (n = 37) was administered with 0.75% levobupivacaine 15 mL. The sensory block effect and motor block effect during perioperation period were observed. Results There were no statistical differences in sensory block onset time and time to the highest sensory block level between the two groups (P〉0.05). Sensory block maintenance time in the dexmedetomidine group [ (194.2-±66.8) min] was significantly longer than that in the control group[ (165.2±69.8) rain,P〈0.05 ]. There were no statistical differences in motor block onset time and maximum motor block distribution between the two groups (P〉0.05). Motor block recovery time of the dexmedetomidine group [ (373±27) min ] was significantly longer than that of the control group [ ( 315 ± 26 ) min, P 〈 0. 05 ]. Maximum motor block time of the dexmedetomidine group [ (22.7±4.6) min] was significantly shorter than that of the control group [ ( 25.7±5.8 ) min, P〈0.05 ]. Conclusion Dexmedetomidine added to levobupivacaine for epidural anesthesia can prolong sensory block and motor block maintainance time and enhance the effect of motor block and analgesia, moreover, the anesthetic effects become more perfect.
